| 1797875 | 2009-05-07 | Viol(s) Noted & Immediately Corrected | At the time of the inspection, the well was in the process of being drilled. Several deficiencies were noted while on-site:
1- There was a steady flow of mud and water coming down the entrance to the access road. The mud was flowing directly on to the township road. 2- There was evidence of off-site transportation of sediment occurring at more than one location. The first location was at the entrance to the access road. There was also evidence of off-site transportation on the lower corner of the well pad. It appeared that the water was running along the silt fence and then flowing under it and down into the natural ravine below. Spoke with Dan Rice while on-site and crews immediately addressed the problems. |
