0399362831 2013-06-24 Orphan No Violations The Division received an orphan well inspection request from the landowners son, Mr. Eli Arbogast. This inspection, conducted with Mr. Arbogast, confirmed that was not orphan but allegedly owned by his father. This well situated behind a rental house at 6289 N. US 23 which is owned by Mr. Arbogast. He stated that the Health Department required plans for the oil and gas well, if they wanted to avoid the house being condemned. During the inspection, it was noted that the well was idle. The well consisted of rods, tubing, and 4.25 outside diameter casing equipped with a wellhead just above ground level. A pumping unit (shackle line) remained attached but was disconnected from a turn table and motor approx 15 away. There was no signs of recent domestic or commercial production. Mr. Arbogast stated that his father used to pump the well with a tractor to get to the gas. The well was plumbed to a homemade cube shapped steel tank approx 50 away (before railroad tracks). The tank dimensions are 66 x 66 x 66 with an open top. There is approximately 1 of fluid in the tank. The ground, 1 out from each side of the tank was stained with weathered oil. Mr. Arbogast confirmed that his father would have fluid removed from this tank and asked about current regulations regarding the handling of this fluid. Additionally, an older style separator (~50 bbl) was found within vegetative cover and had a motor next to it. Note: A second casing was also observed near the well location and measured approximately 5 outside diameter and was filled with dirt. This casing was likely a water well affiliated with the drilling of this oil and gas well.

1392304305 2014-02-13 Production Wells Violations Noted This inspection was done at the request of the landowner, Eli Arbogast, and in conjunction with complaint #3948. At the time of the inspection the well has 4.5 casing with a casing head with no other visible casing. It is presumed that the well was constructed in the same fashion as other historic well in the area and has 8 drive casing and 6 surface casing. The 4.5 casing was likely put in as a liner due to issues with the 6. The well has 2 tubing, rods, and a shackle line pumping unit, but is not connected to any turntable or power source. The well would need servicing if production were to be attempted. A gas line off the 4.5 annulus is run to a temporary holding tank 10 to the south. The two gate valves on the gas line are closed and the tank now has two open holes so no gas remains in the tank. According to Eli Arbogast, the last time the well was pumped was over twenty five years ago by his father, Louis Arbogast, and has not been used for domestic purposes for many years. There is an open topped square tank with approximately 1 of sludge in the bottom 50 northwest that has a small leak and needs to be cleaned up as soon as possible. The well has a Historic Producing status in RBDMS and no records were available. According to the landowner, and a brief search at the County Recorders Office, there are no leases, current or past, for the property. Because the landowner has previously pumped the well and used the well for domestic purposes, they have assumed ownership of the well and needs to have it produced or plugged in accordance with state code. The landowner has indicated the property is being used as a rental, therefore it will not qualify as an exempt domestic well. The landowner, whom was on location for the inspection, was given instructions on getting the well permitted to plug or produce.
1395930571 2014-03-27 Production Wells No Violations This inspection was done as a follow-up to Inspection # 1392304305 done on 2-13-14 and Complaint # 3948. A Compliance Notice was issued on 2-13-14 for failure to register/obtain or maintain bond/insurance (ORC 1509.07) and well shall be plugged (1509.12B). A copy of this notice was requested by the landowner and delivered by the inspector on 3-27-2014. At the time of the inspection the well has yet to be brought into compliance. The well was used by the Arbogast Family in the past as a domestic well, but is currently shut in and disconnected from the residence @6289 N. US 23. The 6 by 6 by 6 , open top production tank is 70 east and has about 18 of oily sludge on the bottom. A trace amount has leaked out the valve located on the bottom part of the north side but is not leaking currently.
1412690333 2014-10-06 Production Wells No Violations This well is associated with Complaint # 3948. At the time of the inspection, Black Swamp Oilfield Services was on location to start the plugging process. A service rig was set up on the well and a 1000 gallon tank brought on location. Several attempts were made to pump the well with the rig before pulling the rods. Each time the rods were lifted they would not go back down indicating the hole is likely mudded up. The well was shut in until plugging operations continue. The leaking storage tank had been removed along with the remaining production equipment last month.
1413470841 2014-10-13 Plug / Plug Back No Violations This well is associated with Complaint # 3948. At the time of the inspection, Black Swamp Oilfield Services was on location and ran 1.5 tubing with a mud anchor to a depth of 1058. The suspected Total Depth of the well is around 1150, but the Trenton formation was caved in around the original tubing and caved in more after it was removed. A bottom plug of 100 sacks of Class A was pumped down through the 1.5 tubing with circulation of fluids to the surface. The bottom plug should fill 550 to 600 of a 6 hole, bringing the top of cement to 500 from the surface.
1415810079 2014-11-04 Plug / Plug Back No Violations At the time of the inspection the last plug was set in the well and the casing was cut off 30 below grade. An identification plate was put on the well and the remaining hole was filled. The area was graded and all remaining production equipment was removed. All violations from Compliance Notice # 1392304305 issued on 2-13-14 have been resolved.
1422984257 2015-02-03 Final Restoration No Violations At the time of my inspection the area had been graded and cleared of all debris. The area had not been seeded and a Landowner Waiver was completed and signed, waiving the establishing of vegetation on location. This will be handled by the landowner as the weather allows. The Landowner Waiver was received on 2-3-14 and the Final Restoration is passed.
1904965990 2014-10-07 Plug / Plug Back No Violations This well is associated with Complaint # 3948. At the time of the inspection, Black Swamp Oilfield Services pulled 1158 of rods, 410 of wooden and 748 of steel. There was evidence of oil on the rods around 200. The weather was rainy and rain was forecasted for the rest of the day so plugging operations were stopped.
