2006341 2011-09-13 No Violations Noted The intent of the inspections was to assure the flowback water did not exceed the 2¿ freeboard requirement as per 25 PA code 78.56 (a) (2) and the liner did not experience compromised integrity as per 25 PA code 78.56 (a) (4), due to recent rainfall events from tropical storms. During the inspection, the freeboard level was observed to be right at the 2¿ freeboard level. Compromised integrity of the impoundment liners was not noted. Jason Petlock of Southwestern informed us that prior to 9/14 the freeboard level was slightly exceeded in the 40,000 barrel capacity flowback water only impoundment; Permit number ESX10-015-0078. Southwestern transferred water from this impoundment to vertical holding tanks in the tank farm area to keep the surface water elevation as low as possible. Some staining and water marks were observed form the surface of the water to the top portion of the liner. The 200,000 barrel capacity flowback/fresh water blended impoundment, Permit Number ESX10-015-0072, did not experience freeboard exceeding and staining.

Sample # 0257070 was collected from the discharge of the small laggon underdrain discharge. Primary focus of investigation is to determine the status of the integrity of the small lagoon liner. Large lagoon liner integrity is not in question at this time.

2099253 2012-09-10 Outstanding Violations - No Viols Req'd Inspection conducted on 9/10/12 with Rich Gulyas. 2 new monitoring wells have been installed. One is located below the small impoundment, and the other is located below well pad and between the large and small impoundment. Drill cuttings and casing sealant debris is located around both new monitoring wells. Rich said he would get rid of this debris ASAP. The sump pump located in the sump cellar which was installed in the rocked swale right above the small impoundment is not functional. Rich not sure why. Please send an email explaining your intentions for this sump pump.
