1994363 2011-06-21 No Violations Noted Site visit was part of routine inspection. Road, pad and pits were under construction.

Heavy rains had compromised some BMPs. HRI (construction contractor) was actively upgrading or adding additional measures to prevent erosion from the site. It was suggested to add diversion ditches and sediment traps to deal with the water from the site. No violations noted at this time.

1994405 2011-06-29 No Violations Noted Site visit was conducted to address landowners concerns. The Kalps, ARM and HRI were represented along with the DEP.

The Kalps were concerned with erosion of the site impacting their property which is located down gradient from the construction. Previous heavy rains had washed some sediment onto the lane adjacent to the site. HRI were to address this as the Kalps requested. The discussion of sed traps and culverts was continued. The Kalps objected. The issue of site access was also discussed with the request of contact with ARM representative prior to site access. No violations noted at this time.

2000746 2011-07-25 No Violations Noted Site visit was part of routine inspection. Mr. Kalp came on site during inspection. No one else was on site.

Visit was after an extreme rain event. The force of the rain water had pushed the socks out of place. Some erosion of sediment had overtopped installed BMPs. Sed trap was full but still in place. Spoke with Dennis Graham about the site and discussed further erosion control measures and the immediate clean-up after the rain. No violations noted at this time.

2012817 2011-10-21 No Violations Noted Site visit was part of routine inspection. Patterson-UTI Rig #480 was on site. Spoke with Joe Ray Company Man on site.

Haliburton was on site. Cementing taking place. BMPs around drill pad and impoundment were sound. Pits had adequate freeboard. Secondary containment was in place. No violations noted at this time.

2026777 2011-12-05 No Violations Noted WELL PERMITED BUT NOT DRILLED. E&S AND PITS, GOOD
2034564 2012-01-11 No Violations Noted Site visit was part of routine inspectiion.

One well was drilled on the pad. Rig had been removed. BMPs in place. A rock lined ditch and sediment trap were being installed on far slope (north) of pad. This will eleviate the erosion issues on this side of the pad. All changes should be red marked on the e&s plan. All other BMPs were in place and appeared to be working properly. Three potential spills were spotted on the pad. One appeared to be an oil based mud. Two others appeared to be a fuel sourec, possibly diesel. These areas need to be exavated, and removed. Spoke to Mike Cribbs (WPX). Water quality specialist Rich Thomas was present during inpection. No violations noted at this time.
