1847093 2009-11-16 No Violations Noted Site inspection and E&S plan review found issues with the utilization of encaspulation and below grade drill pits and the depth to the seasonal high groundwater table. A revised E&S plan was submitted addressing those issues through land farming and elevated drill pits, and additional contingency options should during site construction it be discovered that landfarming and or pits will not be able to be utilized, steel tanks will be utilized and the cuttings will be hauled to a landfill. The remainder of the E&S plan, as compiled, should effectively minimize accelerated erosion & sedimentation, provided the plan is implemented and maintained properly.
1918551 2010-09-24 No Violations Noted Site had been constructed as outlined in the E&S plan. Silt fencing and haybales required replacement. Spoke with operator representative Robert Carnahan onsite and silt sox will be installed on 09/27/10. No accelerated erosion & sedimentation was observed in relation to the well-site or access road. Below grade pit onsite and it had more than the required two feet of lined freeboard. Operator should endeavor to reclaim the site and close the pit upon completion of drilling and any stimulation activities and implement BMP's as necessary to insure erosion & sedimentation is minimized through the fall and winter months.
1918553 2010-10-07 No Violations Noted Drilling appeared to have been completed at the time of the inspection. Silt fencing has been replaced with silt sox for erosion control. No accelerated erosion & sedimentation was observed in relation to the access road or well-site. Below grade drill pit located onsite and it had more than the required two feet of lined freeboard.

2125186 2012-12-21 No Violations Noted ell appeared to be equipped for production at the time of the inspection. Brine tank located onsite and it appeared to be in sound condition. Site had been properly restored and the vegetative cover met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
