Conductor pipe set at 13 3/8¿ to 27.5 ft. Surface run to 1524 ft with 7¿ pipe with290 sks Class A and 16 bbl return. Plug down at o9:53 hrs on 8/17/10. Water set at 295 and 330 ft. Current depth at 3560 ft. Anticipated depth is 4870 ft.
1914054 2010-09-17 No Violations Noted At the time of this inspection, the site properly signed with E&S plan. Well drilled and equipped for production. While onsite, observed the proper installation of filter fence down gradient from well head surrounding lower perimeter of well site. Site had been recently seeded and mulched. Brine tank installed and surrounded by a rock dike, appears structurally sound. It is recommended that operator rock culvert outlets to eliminate the potential for a possible pollution event. No accelerated erosion or sedimentation or IW issues were noted during this inspection. Operator should continue to monitor all bmp¿s until final restoration have been completed and permanent stabilization is observed.

1965743 2011-04-13 No Violations Noted At the time of inspection, well tag present (as well as sign and ES plan) and well in production. Bine tank surrounded by dike appears sound. Site has been regarded (pits removed) and seeded/mulched, with almost 70% vegetative growth observed, recommend operator reseeded and mulch to ensure uniform coverage across site. All temporary bmp¿s may be removed once uniform vegetative cover is achieved. No accelerated erosion or sedimentation or IW issues observed. Access road stable. A follow up inspection will be conducted to ensure final, permanent restoration is achieved.

Brine tank located in containment dike, both appeared sound. Department recommends that Operator continue to monitor and maintain site and access road as needed.

Unable to locate restoration report for this site. On 2/8/13, Operator agreed to re-submit reports.
