BMPs are in place and working adequately. There are no E&S issues at this site. Punxsutawney Hunting Club #25H (37-033-26822) will also be drilled at this site.
1809136 2009-06-25 De minimus Violations Noted Site is still under construction. E&S plan on site, Site ID sign does not contain full well permit number. Conductor pipe set to 40 ft by Coda Rathole. Pipe is approx. 16" in diameter. Pit is being compacted and smoothed. It remains unlined. E&S controls are in place according to plan and there are currently no E&S issues. De Minimus violaions noted for site ID sign.
1814807 2009-07-21 No Violations Noted The following items were noted during the inspection:

-Met with Mr. Dean Lindsey, EOG Consultant and Mr. Jason Craft, Tool Pusher -Driller¿s log up to date. -Drilling contractor, Dallad Morris Rig #8, currently drilling at 975' in sandstone. -Derrick, substructure and BOP appear sound. -Site sign and E & S plan were on site. -E&S BMPs were in place and working adequately. -Reserve pit met regulatory standards, 20 mil liner w/ padding, freeboard at 10¿.

1820322 2009-08-13 No Violations Noted The following items were noted during the inspection:

-Met with Mr. Jim Howell, Drilling Consultant and Mr. Huey, Drilling Foreman -Drilling contractor, Precision Drilling Rig #823 moving in and rigging up to resume drilling at 6,225¿. -Site sign and E & S plan were on site. -E&S BMPs were in place and working adequately. -Reserve pits met regulatory standards, freeboard at 8¿.

1822439 2009-08-13 De minimus Violations Noted Site visited w/ OGI Bruce Jankure. Also spoke with Drilling foreman, Mr. Huey on site. Rig was moving in for deeper drilling on 4H

Upon inspection of the site, the following was noted: 1) on fill corner furthest from entrance there is evidence of slope failure. The area had already been addressed and slope was regraded. no material left the site. 2) pit liner contained multiple slices along top of backside of pit. These slices are above the current water level of the pit, and should remain above the 2ft of freeboard mark. Regardless Mr. Huey will have those tears addressed before they pose any problems. Pit liner extends into shaker area and pit area is designed so that shaker side sits lower than backside of pit. 3) Complete permit number for well #4H has been filled in on sign. site will also contain wells 25H and 34H. 4) vegetation on seeded and mulched ares has grown to about 75% 5) BMPs in place are being maintained and remain adequate. There are no E&S issues on site at this time.

1833424 2009-09-30 No Violations Noted Site inspected with OGI Supervisor, Bruce Jankura. Tom Gear, Toolpusher was present during inspection as well.

-no activity on Punxsutawney Hunting Club Wells 4H or 34H. 25H was currently being drilled by air. -pit has been backfilled and all returns were being discharged into a container. -water was being added to process to decrease dust. it was being seperated from the solids in the waste container and placed into a 500 bbl tank to be hauled off to a treatment facility. solids were transferred to a different container to be hauled off to landfill. -all E&S controls remain in place and working adequately. disturbed areas have about 20% vegetative growth -There are no issues on site at this time
