1877308 2010-04-08 No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. All erosion control BMP¿s appeared to have been implemented properly. Noted that earthmoving activity was ongoing adjacent to the drill pad. Spoke with operator Matt Barch via phone regarding this matter and he stated that this was work to construct the frac pit and that an ESCGP-1 covering this activity was obtained and was with the contractor performing site construction at that time.

1918459 2010-10-06 No Violations Noted At the time of inspection, no E&S or IW discharge issues at the site. Pit aligned with proper plastic and no freeboard issues. Keystone Vacuum was removing fluids from pit. E&S controls were in place. Chief was in the process of flow back and flaring gas well from Fracking procedure. Due to Chief policy of ¿fire retardation suites¿, DEP was not allowed to enter the site pad but was able to view the flare and site conditions from the Security station. Based on visual view of the site, the flame was showing no smoke or air quality problems. The site showed no signs of stress vegetation or air quality problems. DEP called Chief (JD Slater ¿ Compliance Manager). He reviewed the log for Sunday and showed no reports of site air quality concerns.

Observed large amounts of sand atop pad. No pits on site, only frac tanks. No observation of industrial discharge. Operator actively repairing a ¿minor¿ leak on well head during our well inspection. Well fraced in Aug and recently burnt off (flared). Site was being monitored by security 24hrs a day. Accompanied by WQS Dana Burk, and WQSS Ed Ritzer.

Well showing 3000 psi at wellhead and was connected to condensation unit followed by 2 ¿ 100 bbls brine tanks. Both units located in a containment area. DEP representatives present were Paul Wyrwas and Marc Maestra. Marc inspected the site pad and perimeter of the site pad and found no issues.

Well drilled and in production Two brine tanks located on site located in containment unit appear sound. No IW issues noted. Previously disturbed areas restored with adequate vegetative cover. Pad adequately rocked. No accelerated erosion or sedimentation issues noted onsite or access road. Noted large rock pile on back side of pad, recommend operator remove in order to prevent a possible pollution event. No violations noted at time of inspection.

Site gated from third party access (, but pipeline installation ongoing during inspection). Access road stable, no accelerated ES issues. Well pad adequately stabilized with rock. Drilling completed 05/06/10. Two brine tanks located onsite and surrounded by containment unit. Disturbed areas have been seeded/mulched operator may need to reseed/mulch to achieve uniform vegetative cover; no accelerated erosion or sedimentation issues noted. Temporary bmp¿s are in place and functioning; operator should continue to routinely monitor/maintain all bmp¿s until all earth disturbance activities been completed and final restoration (according to regulation) is achieved. No industrial waste issues observed.

The Department notes that the well pad area and the access road show no sign of accelerated erosion. Site has one well currently drilled and in prodcution. Current operator, Chevron is actively "removing" a drill pit previously encapsulated by former well owners. Chevron chose to remove previously encapsulated pit offsite prior drilling additional wells on this location.

 At the time of this inspection, it appears that site and previsously distrubed areas have been stablized, aside from pit, no accelerated erosion or sedimentation issues noted. ES plan implemented and maintained. 

Reccomended that operator placed excavated pit materials on/in containment structure while awaiting to be loaded into trucks for disposal. 2nd dept also reccomended that operator ensures trucks are cleaned prior to returning with clean fill for reclamation in order to prevent a possible cross contamination pollution event

Bmp's implemented and maintained. Chevron removed a drill pit previously encapsulated by former well owners. Chevron chose to dispose of previously encapsulated pit offsite prior to drilling additional wells on this location. Pit contents and liner have been removed offsite, operator agreed to supply department with manifests for all materials removed from pit (including liner)--operator unable to account for liquid removal prior to encapsulation as it was handled by previous owner. Operator also has plans to file an Amended Restoration Report to indicate that the previously encapsulated pit has been removed off site. Based on current observation, dept concurred w/ opr proceeding w/ plans to backfill pit, it visibly appears contimanted soils/materials removed
2078166 2012-06-20 No Violations Noted Inspection result of operators notification that previously encapsulated pit has been successfully removed and backfilled. During today's inspection, department noted that area of previously encapsulated pit had been back filled with clean fill material, but due to recent heavy rains it appears that soil has "settled." Due soft nature of the fill it was recommended that operator add additional fill prior to rocking area to ensure that material is properly compacted and stabilized as operator has plans to drill another well on location and drill rig may be placed over area recently back filled. Again, department is currently awaiting final response from operator including disposal records, amended restoration report, as well as written report on activities undertaken for removal of previously encapsulated drill pit.

The Department suggest the Operator continue to monitor the site weekly and after significant precipitation event to ensure the stability of the site is maintained.

Site ID sign, valid permit, E&S plan, ESCGPI, PPC Plan present at access road entrance. Security on-site. At the time of this inspection, 1H had been drilled. Actively drilling 5H, cellars for 2H,3H,4H constructed, 6H staked. Minor erosion on hillside observed on back left corner of pad when facing wellheads from access road. Minor sediment from this erosion observed at base of hill in rock-lined ditch. Observed tophole cuttings along soil pile. Department spoke with company representative who stated plans were to incorporate cuttings into soil pile and disperse across pad. Operator will address erosion concerns by routinely monitoring this area in order to prevent pollution event due to accelerated erosion and sedimentation. Access road adequately rocked and no E&S issues were noted in relation to access road. Department recommends that Operator continues to monitor and maintain access road and well-site as necessary.

2136131 2013-01-31 In Compliance with Policy Admin Ispt. Spoke to Chevron representative Curtis Small via phone on 1/31/13. According to Mr. Small, on 1/30/13 while HWR company was loading excavator parked where access road branches off towards impoundment, hydraulic line burst, spilling approximately two quarts of hydraulic oil onto access road. Spill was immediately soaked up and the impacted gravel shoveled into drum. Samples taken, will dispose of in landfill. It appears situation was handled according to policy and a follow-up inspection will be conducted in future to ensure full compliance.
