Cement returns to surface not realized with plug landing at 15:30 02-04-11. Estimated that cement 10'-15' from surface. Will grout backside when cementing 11 3/4" casing.

Note- Some cuttings have blown out of the pit onto the pad. The cuttings will be placed in the pit. The operator has utilized a flange to prevent this from happening again.

Cement not returned to surface. Drilled 6 1/4" hole to approximately 3150' prior to plugging back to approximately 2400' for proposed kick-off point of 2440'. Cement bond log to be run on 02-17-11. Drilling pit lined and freeboard maintained. No E/S control issues on site. Inspection conducted w/OGI J. Branan.

Titan Wireline ran cement bond log in 8 5/8" casing. Tagged cement at 2388'. 8 5/8" casing shoe at 2377'. Log showed 50% bond of 8 5/8" in 11 3/4" casing at 250'; 90% bond at 400'. 509' of 11 3/4" casing previously set in 15" hole and cemented with 260 sacks of "A" containing 3% CACL2 & 1/4# flake & 0.2% foam chek (yield=1.18 cu ft/sk). Cement returns to surface realized with plug landing at 01:00 02-05-11. Drilling pit lined and freeboard maintained. No E/S control issues on site.

1998540 2011-08-25 No Violations Noted Drilling had been completed, well appeared to be equipped for and in production. Oil and brine tanks located onsite and they appeared to be in sound condition. There was no accelerated erosion or sedimentation observed in relation to the site or access road. The site had been properly restored and the vegetation had begun to grow. Upon achieving 70% uniform vegetative cover the operator should remove the temporary silt fencing erosion controls.
