Well Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1975194 2011-05-20 No Violations Noted Site inspection performed as a result of phone conversation with Ted Parrish regarding additional erosion controls related to capturing and conveying springs so as to insure the stability of the constructed well pad and to minimize accelerated erosion & sedimentation. In addition the site construction contractor had voluntarily ceased earth moving activities due to the weather conditions and saturation of the soils. Contractor stated that the piping for the springs and remaining sediment trap construction will begin in earnest Saturday May 21st. Observed no evidence of accelerated erosion or off-site transport of sediment. All erosion controls had been implemented so far in line with the E&S plan included with ESCGP ESX10-005-004.
1981431 2011-06-17 No Violations Noted Construction was underway for the second of the two impoundments to be located onsite. The reminder of the site had been constructed and all E&S controls had been implemented as outlined in the ESCGP-1. No accelerated erosion or sedimentation was observed on the well-site or access road. The operator should inspect the erosion controls and sediment traps as outlined in the ESCGP-1 to insure their continued functionality.
2001552 2011-09-08 No Violations Noted Drilling was ongoing at the time of the inspection. Inspection found a minor erosion issue related to the area below the impoundments. Consulted onsite with operator representative Ted Parrish and he informed WQS Higgins that an additional filter sock will be added to the two current socks in pyramid fashion, that additional rock will be added to the low point below the impoundments, in addition to seeding and mulching to provide temporary stabilization. In one of the well stimulation impoundments located onsite a small bubble was observed. There was minimal water observed in the pit and this was attributed to precipitation. Informed Mr. Parrish of the Department¿s concerns about this bubble, and suggested that it would be better to evaluate this issue and address it at this time than to let it prolong and possibly grow into a larger problem as the onsite impoundment is put into use. Requested that Mr. Parrish contact WQS Higgins via phone once EXCO has decided upon a course of action to address the impoundment issue.
2008789 2011-07-28 No Violations Noted Construction of the site had been completed. Drill rig was in the process of being moved onsite and this should be completed within the next few days. One of the impoundments had the felt liner placed. All of the erosion controls had been implemented as outlined in the renderings in the ESCGP-1 for the site. No accelerated erosion or sedimentation was observed while onsite. Noted that vegetation had begun to take hold on the slopes associated with the site.
