The pit liner is embedded properly and the water level is within the freeboard requirements. There are no signs of erosion and sedimentation on well site or access road. The operator should continue to monitor the filter socks to ensure that they are continuing to function, until final restoration activites are completed. This is a MARCELLUS WELL.
1821143 2009-08-13 No Violations Noted The well site has a series of CO2 tanks on site in preparation for a gas frac. There are no signs on erosion and sedimentation on the access road and well site at the time of the inspection. Drilling is complete.
1822462 2009-08-17 No Violations Noted DEP inspected well sign, E&S plan and access road to well head. Met Samson representative, Charlie Howard. Subcontractor, BJ was at the site performing set up of the CO2 frac. Multiple tanker trucks of CO2 and Nitrogen along with pump trucks were on-site. Barriers were aligned in an acceptable manner. No Freeboard issue with the pit and Frac water pit was built. Confirmed frac time of 8/18/09 at 7:00 hrs and departed site.
1822471 2009-08-17 No Violations Noted DEP personnel, Paul Wyrwas, Herb Karlinsey, Chad Eisenman, and April Wieland. DEP inspected well sign, E&S plan and access road to well head. No Freeboard issues with both pits and pits are aligned with 30 mil plastic. Samson representatives, Charlie Howard and Steve Trujillo. Subcontractor, BJ was at the site to perform the CO2 frac. Multiple tanker trucks of CO2 and Nitrogen along with pump trucks were on-site. Safety meeting was held on-site prior to frac. Test lasted for approximate 30 minutes for nearly 6000 psi before mechanical failure occurred. BJ and Samson were going to attempt a second CO2 frac once the well was purged.

Pad in place; no production equipment installed yet; perimeter BMP¿s still in place and functioning adequately; site revegetated with minimum 70% vegetative cover. Access road gated, maintained and no E&S problems noted.
1965626 2011-04-14 No Violations Noted Well actively being plugged. Access road appears stable. All pits have been removed, areas seeded mulched with >70% vegetative growth. Temporary bmp¿s in place functioning, operator may remove upon final, permanent restoration, which under regulation is to be established within nine months from the completion of plugging. No accelerated erosion or sedimentation issues noted on site or access road. Spoke with Tom, of Samson Resources, who stated plans have not yet been decided on how the site will be restored, department reiterated that according to regs >70% uniform perennial vegetative cover or other permanent stabilization measures that are capable of resisting erosion are required for permanent restoration. Operator has plans to utilize an erected monument, which is ok by department. Any agreements on specific restoration plans, are handled between operator and land owner and are considered to be part of lease agreement and are not handled by the department. Follow up inspection will be conducted to ensure permanent restoration.
1986408 2011-06-30 No Violations Noted Site visit was conducted to discuss restoration activities. Bobby Pugh represented Samson Resources.

Well had been plugged with stack and vent. Welded plate marked the plugged well. Site was built on a moderate slope. Pad itself was level. The toe of the pad was well vegetated, showing no erosional tendencies. .It was agreed upon that the best restoration would be to leave the area with its current configuration. Stone would be removed and adequate topsoil would be added. Seeding and mulching would then be required. No violations noted at this time.

1996483 2011-08-04 No Violations Noted Site visit was requested by land owner Leroy Troutman. PA DEP O&G Inspector Paul Wyeras was also present.

Mr. Troutman continued to voice his displeasure with the restoration and plugging of well #42-111-20271. He was displeased with the above ground vent pipe. A copy of the regulations was supplied explaining the reason for it. Secondly, he was unsatisfied with the regarding of the site. The toe of the slope was left due to the proximity of the plug well. Also, the stability of the current slope along with vegetative cover already in place was deemed viable for restoration. The slope of the toe was not severly steeper than the slopes that were undisturbed before construction of the pad.

1997391 2011-08-04 No Violations Noted DEP (Mark Maestera and Paul Wyrwas) met with Mr. Troutman and discuss the site restoration and well monument. DEP confirmed that the monument was completed as per regulation. The site has been restored under 78 regulations and the slope of the site pad can not be regraded due to stabilization of vegation and concern of disturbing the structural integrity of the abandoned gas well.
