Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1794193 2009-04-16 No Violations Noted Well site appears to be in production. No evidence of accelerated erosion and sedimentation noted on the access road. Brine Tank located on-site is in sound condition. Several concerns were noted during the inspection:

1- The old silt fence is still on-site. The area is well vegetated and the silt fence is no longer needed, so it should be removed. 2- There is an area adjacent to the brine tank where the ground is bare. Seeding and mulching the area is recommended.

1894873 2010-06-15 Violation(s) Noted Johnathon Longwill and Adriene Smochek from Representative Dave Reed's office on site during inspection.

Well tag in place. Access road stable, with no signs of accelerated erosion. Site is stabilized with adequate vegetation. No signs of accelerated erosion on site. Inspection revealed the following deficiencies:

 1) Discharge of brine to the ground. - Fluids were found to be leaking at a union joint on the pipe between the seperator and the
production fluids tank. Fluids were found puddled under the leak. Moist areas with dead vegetation began at this point and continued
down the site to just below the tank. Source of leak needs corrected, contaminated soil must be removed, and affected areas limed,
      fertilized, and seeded/mulched.

1904553 2010-08-04 No Violations Noted Well tag in place. Well is equipped for production. Tank appears sound.

Access road stable, with no signs of accelerated erosion. Site is stabilized with adequate vegetation. No signs of accelerated erosion on site. NOV issued on 6/30/10 for discharge of brine to the ground. Compliance evaluation inspection revealed the following: Joint on pipe to the production fluids tank has been repaired/replaced. No signs of additional leak found. Area of discharge below the joint has been filled with rock. Contaminated soil from discharge has been removed, and replaced with fill material. This small area has been seeded and mulched. Site is in compliance. No further violaitons found.
