PIT LOOKS GOOD. ROUGHNECKS WERE LUBRICATING THREADS ON PRODUCTION CASING WITH PLASTIC ON THE GROUND TO CONTAIN ANY EXCESS, BUT HAD A VERY MINIMAL AMOUNT OF LUBRICANT RUN OFF THE PLASTIC ONTO THE GROUND. CM DAN HAD THE CREW CLEANING UP WITH BIO-DEGRADEABLE OIL SORBENT WHEN I GOT THERE. THE CREW IS DOING A FINE JOB CLEANING UP. RIG MOVING TO 6-H NEXT

The liner had been torn and the hole was approximately 6 feet long. The pit will need to be drained and its contents will have to be handled in accordance with Department regulations. The well site was prepared within 100 feet of a stream identified on a

7 1/2 minute USGS map. The Operator failed to obtain a waiver for the 100 ft distance restriction.

37-059-25279 ON PAD WITH 37-059-25313, 37-059-25277, 37-059-25278, 37-059-25347, AND 37-059-25280. BRONCO 37 DRILLING AHEAD ON 4-H @ 9850 WITH 2 WELLS COMPLETE AND 3 WELLS TO GO ON THIS PAD. CM RICKY SAYS EVERYTHING IS GOING WELL. SITE IS CLEAN, BACKYARD NEEDS SOME ATTENTION. CARSON WAS ONSITE TAKING CARE OF HOUSEKEEPING ISSUES. PIT LOOKS GOOD, ECA WAS SHUT DOWN FOR A FEW DAYS FIXING A PARTED SEAM IN THE PIT LINER WHICH MAY HAVE BEEN CAUSED BY BUILDING PITS ON THE FILL SIDE OF LOCATION. PREVIOUS WET WEATHER MADE DETECTING THE LEAK DIFFICULT, BUT HILLSIDE IS DRY NOW AND NO LEAKS ARE VISIBLE. RIG IS CLEAN AND SITE LOOKS GOOD.

1884637 2010-04-14 No Violations Noted The Department was at the above referenced Marcellus Well to conduct a CEI Inspection. The Department issued the Operator a NOV on 3/5/10 in regards to the pit leak and the waiver for the 100 ft distance restriction. The Operator has repaired the liner and is currently utilizing the pit for drilling. An unknown amount of drilling fluids leeched into the soil for an unknown amount of time. The Department discovered the leaking pit. The Operator failed to properly notify the Department. The Department has not received the waiver for the blue-line tributary to Muddy Creek. The Operator has disturbed soil and encroached on a stream without a permit (Less than 10 feet from stream) as mentioned in the 3/5/10 NOV.

There are other issues at this site and will be addressed in another report (Coal seam was encountered, acid rock drainage).

1884651 2010-04-14 Pending The Department observed some environmental concerns while conducting a CEI Inspection at this site. These issues will be considered different from the 3/5/10 violations. The Department observed 2 acid rock seeps originating on the fill side slope. The Operator had contacted the Department because they encountered coal while excavating the impoundment. DEP Mining program was contacted to deal with the coal issues. The acid rock drainage will need to be addressed.

The acid rock seeps on the fill side slope continue to drain down the hill. The Operator placed bags of soda ash in the channels to control the PH and catch the iron. The Operator is responsible for maintaining the treatment of that water. Inspections should be made on a regular basis to control the contaminant levels. The Department will continue to inspect these acid rock seeps. The inspection results are pending.

The Department received a call claiming that the groundwater has been impacted down gradient of this site. The Department will contact the residents to schedule a sampling event. The acid rock seeps on the fill side slope continue to drain down the hill. The Operator placed bags of soda ash in the channels to control the PH and catch the iron. The Operator is responsible for maintaining the treatment of that water. Inspections should be made on a regular basis to control the contaminant levels. The Department will continue to inspect these acid rock seeps. The inspection results are pending.
1889715 2010-05-25 Pending The Department was at the above referenced Marcellus Well to conduct a follow-up inspection in regards to the acidic water seep on the fill side of the site. The Operator is currently drilling multiple Marcellus Wells on this pad. Coal was encountered when constructing the water impoundment. They disposed of the coal. The drilling pit contained excessive oil during inspection. The sediment trap and E/S controls need cleaned-out. The inspection result is pending. The Department will sample the water wells down gradient of this site. The Department received three complaints in regards to impacted water supply wells.

BRONCO 37 COMPLETED ALL WELLS ON THIS PAD. THEY ARE CURRENTLY CLEANING OUT MUD TANKS INTO THE PIT BUT ARE ALMOST OUT OF FREEBOARD. I'VE ADVISED ANDREW THAT HE NEEDS TO HAUL OUT WATER IMMEDIATLY TO AVOID A FREEBOARD VIOLATION. BACKYARD HAS BEEN CLEANED UP, EVERYTHING LOOKS GOOD. THEY ARE STILL MONITORING AMD SEEP.

1896930 2010-06-08 No Violations Noted The Department was at the above referenced Marcellus Well to conduct a routine inspection. The Department was following-up on the process of the fresh water impoundment and the iron seep on the drill site. Orange acid/iron contaminated water continues to seep out of the fill side of the site. It appears that 3 water supplies have been impacted down gradient. Water samples were taken at these locations and the Department will discuss options with the Operator to resolve the impacts if necessary.

2047779 2012-01-31 No Violations Noted The Department was at the above referenced Marcellus Well to conduct an inspection. The Operator has drilled 6 wells on the pad. The wells are drilled and in production. There is an on-going water quality investigation at this site. The site has not been restored at this point but the Operator will begin work next week. The E/S controls are in working condition and temporary site stabilization has been obtained. There is no water pouring out of the fill side of the pad during this inspection. This is where the orange acidic water poured out of the pad. The Operator will determine how to control this water.
2052327 2012-03-02 No Violations Noted The Department was at the above referenced Marcellus Wells to conduct an inspection. The Operator has drilled and fracked the wells and they appear to be in production. There is an ongoing water quality investigation taking place at this site. The Operator hired Moody to install ground water monitoring wells to evaluate the ground water. The Operator was on site restoring a water impoundment. The site will need to be seeded and mulched after final grading.
