Appalachian performed cement using 170 sks of Class A. Water located at 248 ft. Bit snapped. Well was cemented back with 100 sks Class A to 325 ft.
1953989 2011-03-02 No Violations Noted site ID sign and ES plan present. Rig onsite drilling. Observed the installation of compost sox down gradient from well location. No accelerated erosion or sedimentation issues noted in relation to well site or shared access road. Well being drilled on former strip location and current new enterprise site. One below grade pit located onsite surrounded by security fence. Pit appears adequately lined with ample freeboard. Cuttings are to be disposed of by land application onsite. Observed evidence of a minor spill/release (appeared < 5gals of ¿oily¿ substance) on small puddles behind rig. Spoke to Joe of US Energy onsite, who was unaware of any ¿releases,¿ but as requested absorbent pads were placed in the area of concern to clean up ¿release¿ and it was also recommended that operator monitor production and construction equipment for possible leaks, as no evidence of a active leak could be identified at this inspection.
1954878 2011-03-03 No Violations Noted Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Pit covered with 30 mil plastic and security fence. No freeboard issue. U.S. Energy was contracted. Stratigraphy consisted of coal, ss and sh. Surface casing (7¿) ran to 1569 ft with 265 sks cement with 15 bbl return with plug down at 05:00 hrs and waiting on 8 hrs time. Slight petroleum release around rig. Absorbent pads were utilized but release was not petroleum base and was not absorbed by pads. Appalachian well service used for cementing. class 1.
1965651 2011-04-13 No Violations Noted At the time of inspection, well drilled, equipped and in production. Site Id sign and Es plan present. Well surrounded by concrete barriers to prevent a possible accident as well located on former strip located and current new enterprise site. Brine tank surrounded by dike appears sound. Drill pit has been removed; therefore, pad and surrounding areas have been adequately rocked (appear capable of resisting erosion) as site was not vegetated prior to disturbance. No accelerated erosion or sedimentation or IW issues noted during inspection. Recommended that operator continue to closely monitor site in order to prevent a possible pollution event due to high traffic location.
