-1096358384 2009-04-22 Site has been prepared for drilling. Trees have been cleared and pushed into piles. Stone base access road goes down slope to well site. Water was flowing down hill next to road and has caused some erosion and sedimentation. Well will be near a creek. Spoke to John Suhar by phone and recommended straw bales or silt fence for site.
1240852040 2008-04-27 Call from operator about artisian flow of water at depth of about 85 feet. Drove 92 feet of 16 inch pipe into a 20 inch hole and cemented around bottom of pipe. Next drilled 15 inch hole to depth of 164 feet and cemented 158 feet of conductor pipe. Water continued to flow on outside of 16 inch pipe. Operator dug trench approximately 6 feet deep and diverted water to ditch that discharges to creek. Area around 16 inch pipe was also dug to same depth. Gravel was placed around well casing and in trench, then 8 inch PVC pipe placed in trench to control flow. Trench was backfilled. Water again rose to surface around 16 inch pipe. Stone and larger rock along with bentonite was added around 16 inch pipe. Area around 16 inch pipe was capped with cement. It appears as though water has been contained to flow out through 8 inch PVC.
1241629932 2009-04-27 Received call from Don Kreager of David Hill Inc. While drilling conductor hole an artesian water flow was intercepted at about 85 feet depth. The hole was reamed out to 20 inches and a 16 inch casing was cemented in the bottom of the hole at depth of 97 feet to control the water inside the pipe. Stone was used to backfill the annulus side of the pipe. A trench was excavated about 4 feet in depth to allow a 8 inch plastic drain to be placed to remove the water from the wellhead. As the stone reached near the surface bagged clay was added to the stone. The stone and clay dropped several times during the process. Cement was poured at the surface to seal off water from leaking around the wellhead. Water from the drain at the well site discharged into a ditch near the creek.
1247169482 2009-07-09 Pits have been backfilled and site has been rough graded but topsoil has not yet been replaced. Several pieces of equipment (water tanker, half tank, excavator and semi with trailer) remain on site. It appears as though work still to be done on well before production. Water discharging from pipe into ditch from artesian flow from well. Trees windrowwed along edge of disturbed area are supposed to be buried.
1270060692 2010-03-31 Area around well has been graded and revegetated. Trees and stumps have been pushed in windrow along tree line. Wood appears being cut for firewood. Plan indicates mulched or buried with landowner approval. Spoke to Don Kreager by phone. Well will be plugged in next few months. Will check with landowner for decision on final plan for disposal of wood and will obtain written consent. Also need to get consent to leave stone surface road to well.
1337617914 2012-04-25 This well has never been put into production and needs to be plugged. A plug permit was issued and then expired in April of 2011. I spoke with a company represetative who indicated they were applying for another plug permit to plug the well this summer.
1402061161 2014-04-11 At the time of the inspection the well is not in production and never was put into production. A permit to plug the well expires on 9-4-2014. I spoke with David Hill who indicated that Knox Energy, Bill Mitton, is contracted to plug the well before the permit expires.
192925173 2009-05-14 All equipment on site. Pits have been backfilled. Large piles of trees and brush on site. Some sediment in ditch at tile discharge but has not reached the creek.
259108517 2011-08-18 Well shut in. Some gas odor. No production equipment on well. 10' x 10' plastic around wellhead and taped to 4.5" casing. Will contact operator regarding status of well.
943433085 2009-04-30 Picked up cement tickets from surface casing cementing at 1:00 AM 4/30/09. Spoke to operator on site. No problems with cementing. Drilling has resumed and are now 4 joints below the bottom of the surface casing.
