1955583 2011-03-08 Violation(s) Noted Today Jon Scott I completed a inspection of this site in response to a complaint received 2/28/2011 alleging the Glavic # 1 gas well was impacting the drinking water source. Upon arriving at the site I meet the complainant and completed the Water Supply Investigation Checklist. Upon investigating I found the gas well site partially reclaimed, being unstable. The site had both surface water and ground water flowing across bare earth, that had pieces of pit liner, E&S fence, concrete and bentonite mixed in to the dirt. This water was pooling along side the access road, and several hemlock trees are dying in the area the water has pooled, and in the area of flow along side of the access road. The complainat's water source appears to be in line withe water flow; I suggested to the home owner that he not drink his water until we verify if it is safe. I collected a water sample for a 942 series analysis, to determine if the gas well is impacting his source. I notified Kastle's Project Manager Scott Tipton, and setup a on-site meeting for 3/10/11. 606136
1956598 2011-03-10 Violation(s) Noted My inspection today was to visually check to see if water was running from the site west across the access road into the stream. Upon inspection I found the water continues to run across the site carrying sediment and possible waste products, and pooling along the east side of the access road. The water was not overflowing thr access and running into the stream. Violations of 102.11 remain. I collected (3) water smapel to be run under the 942 series. One from the site and the others from pools of water that has run off the site. More info in my 3/10/11 inspection. 606371
1958930 2011-03-17 Pending Today Scott Lux and I met on-site with Kastle Rep¿s Scott Tipton and Kevin Saffell to review the apparent pollution event occurring in the area of this well site. Upon examining the site and surrounding area, it was agreed that some type of pollutant is running off the site in the water and impacting/killing the trees that are in the flow path. The current thoughts are that when the site rock material was removed/reclaimed in August 2010, contamination in the area was ¿stirred-up¿ and then flowed out of a tail drainage ditch (constructed at the same time) to the west and pooled along the access road. The pooled water then seeped through the ground and rock in the area flowing primarily to the south, towards the Sabol residence, impacting several trees along the way. Mr Sabols¿ spring water source is directly in-line with the water flow. A drinking water sample was collected.See my 3/17/2001 inspection report for more information.
1991383 2011-07-26 No Violations Noted Met on-site with Owner and sub contractor to review and discuss drill pit clean-up options. Explained to the owner/operator that he would either have to pump all fuilds out of the pit and take it and the liner to an approve waste site or dig a new pit and place the old liner and pit contents in it. I also explained i would ahve to see the old pit, after removing the material and before he back filled it.
2002078 2011-09-08 No Violations Noted Inspection to verify if the site reclaim work has been completed- upon inspection i found the gas well site reclaimed- with 90% of the ground suface cover with clover/grass- the access road is in good condition- the drainage culvert has been installed as required.
2004625 2011-09-21 No Violations Noted Today Scott Lux and I inspected this reclaimed site to determine if the restoration is complete. Upon inspeciton we found the site reclaimed as required. We did notice two small bare spots on the west side of the reclaimed site. Theses sies will be watched to see if the grass comes in as required.
2007338 2011-09-28 Pending Today's inspection was to check on the status of the revegation of the barren areas of this reclaimed gas well site and to review the results of water and soil samples I had collected, with property owner Ken Sabol. Upon isnpection I found the barren spot still did not have grass growing. I contacted Scott Tiption, of Kastle Resorces and explained that Kastle would have to remove the soil from this area and replace it with clean top soil, seed and mulch this fall or early next Spring. I will have to reinspect.
