Water storage impoundment being lined. Pits were lined and in place. Entry was asphalt and gravel. Roads throughout site were durable stone. Sed trap was full of water. Filter fabric and socks were holding water after sed trap. No sediment leaving site. Other E&S controls installed and working properly. No violations at this time.

1985346 2011-07-01 No Violations Noted Inspection was part of routine site visit. Met with company man and tool pusher. Tool pusher accompanied inspection.

Drill pits contained cuttings and fluids. Although they were below freeboard limits, their levels were discussed with company man. Slopes of impoundment had not yet been seed or mulched due to the ongoing intellation of the liner. The sed pond also needed to be addressed. This was discussed with Dennis Graham (ARM) who stated it would be dealt with asap. No violation noted at this time.

RIG NOT MOVED ONTO PAD YET E&S, PITS, AND HOUSE KEEPING GOOD

1996476 2011-08-02 Pending Inspection was in response to company man¿s report of foam in under drain leading to sediment trap.

Company man felt that some soap foam had landed on the pad and with the heavy rains washed it into the under drains and into the trap. Water was actively flowing from pipe at time of inspection with soap foam along rocked ditch. Water had chemical smell. Sample 0629723 was taken from the outfall of the pipe. Remediation plans were discussed with company man along with multiple erosion issues. No sediment had left site. Also spoke to Dennis Graham concerning erosion issues.

1998264 2011-08-04 No Violations Noted HORIZONTAL MARCELLUS

H&P RIG 287 DRILLING ON WILLIAMS PAD, DEPTH 2000' E&S, PITS, AND HOUSE KEEPING GOOD

2000753 2011-07-28 No Violations Noted Inspection was in response to company man¿s (Anton Francois) report of oil based mud spill.

Spill was caused by a faulty valve on tank. Majority of spill took place on containment. Some mud had been tracked onto stone ramping. Discussed with him the removal of this stone and clean-up of other mud issues. He stated it would be cleaned up immediately. No violations noted at this time.

2008858 2011-08-30 Pending Site visit was part of routine inspection.

Pits were mostly dewatered. Equipment was on site to remove cuttings and mix them with fly ash. The fly ash was piled without containment next to the pits. A pumper truck was removing fluid from hole dug in pad to catch ¿soapy¿ water discussed and sampled on 7-28-11. No fluid was leaving site from that corner of the pad. Frac tanks at corner of site were sitting on compromised containment. Drilling mud and oil were spilled on containment with clean-up pads strewn about. Randy Reimold from Williams arrived on site during inspection. We discussed fuel spill at corner entrance of pad. Some holes were dug to check for fuel oil migration. No sheen was visibible on surface of water. Soil and rock in area smelled strongly of fuel oil. Discussed possible clean-up, sampling, etc. options. Will contact in the morning concerning how to proceed. On 8-31-11:

Spoke to Randy Reimold to discuss plan to set up sampling protocol to determine the extent and amount of impact.  Mr. Reimold stated he would submit a 
2008861 2011-09-19 No Violations Noted Inspection was follow-up from 8-30-11 site visit.

Pipeline had been installed on pad. Frac tanks and their compromised containment had been removed. Underlying stone appeared visually clean. According to phone call with Randy Reimold, fuel spill area had been sampled and came back within limits. Results will be sent asap. No violations noted at this time.
