1776400 2009-03-13 Pending According to Oil & Gas Inspector documentation drilling began on 04/29/08. At the time of the inspection the well appeared to be equipped for production. 1)Noted that the site had not been regraded, seeded, and or mulched. The filter fence located onsite appeared to still be intact and functioning properly. 2) Noted that a below grade pit was still located onsite. The pit had more than the two-feet of lined freeboard.

At this time the Department is requesting that the operator forward a copy of the Well Record/Completion report showing when drilling was completed at the well-site. This documentation should be forwarded no later than April 9, 2009.

1785664 2009-04-21 No Violations Noted ound that the below grade pit which had been located onsite had been removed, and the site had been regraded, seeded, & mulched. Noted no accelerated erosion & sedimentation in association with the well-site or access road. The operator should continue to monitor the site to insure that the 70% uniform vegetative cover is achieved.
