Conductor pipe was installed to 24 feet (13 3/8¿ pipe) and sanded in. Water Protection pipe was installed to 295 feet (9 5/8¿ pipe). Water protection pipe was cemented with 120 sks of Class A with 10 bbls return. Intermediate Casing was installed to 1524 feet (7¿ pipe) and was cemented with 280 sks of Class A with 10 bbls return. Water was encountered at 70¿, 210¿ and 615 feet. U.S. Energy completed the well depth to 5035 feet and was removing drilling rods in order to run the wire line study. Centralizers were set at 127¿ and 258¿ on the 9 5/8¿ pipe and 180¿, 348¿, 516¿, 684¿, 852¿, 1020¿, 1288¿, 1356¿, 1524¿ on the 7¿ pipe..

Site Id sign ES present.Pad appears muddy, additional rock is recommended in order to minimize mud carried on shared driveway as well as township road. Well drilled and equipped for production.One brine tank surrounded by earthen installed. One lined drill pit located onsite. Pit contents drill cuttings and frozen liquid fraction, during this inspection a section of liner had fallen into pit as a result required 2ft of lined freeboard is exceeded it is recommended that operator promptly pump pit as it appears there is the potential that upon thawing pit contents (liquid & drill cuttings) will exceed liner and directly contact grounds of the commonwealth; therefore, operator shall ensure that pit freeboard is restored and/or pit properly disposed of according to regulation. According to plans operator plans to handle drill cuttings onsite according to regulations Bmps installed, shall maintain until final restoration. 630795

2046471 2012-02-23 Outstanding Violations - No Viols Req'd At the time of this inspection, site id sign and ES plan onsite. Spud 11/22/11. Inspection result of violation issued for failure to maintain freeboard and maintain structurally intergrity of liner. During this inspection, it appears that operator placed a section over liner overtop of the area that had previously fallen into pit. It does not appear that liner was properly seamed, as a result previously issued violations are oustanding. As operator did not properly re-line pit according to regulations the potential still exist for pit failure. Department is again reccomending that operator remove and dipose of pit contents in accordance with regulation in order to prevent a potential pollution event. Drill pit removed; therefore, it appears operator has addressed outstanding pit violation. According to ES plan onsite, operator had plans to dispose of pit "onsite according to regulations." Department plans to contact operator to determine which method of disposal was utilized. Site not yet restored to final grade nor has site been seeded/mulched. Bmp's installed, Operator should continue to closely monitor and maintain all bmp's until final permanent restoration is achieved. Minor ponding of water located along access road near well site. Operator shall continue to properly grade well site in order to sustain adequate storm water drainage. Opr written response recieved 2/10/12
