Inspection result of violation issued for failure to maintain freeboard and maintain structurally intergrity of liner. During this inspection, it appears that operator placed a section over liner overtop of the area that had previously fallen into pit. It does not appear that liner was properly seamed, as a result previously issued violations are oustanding. As operator did not properly re-line pit according to regulations the potential still exist for pit failure. Department is again reccomending that operator remove and dipose of pit contents in accordance with regulation in order to prevent a potential pollution event. Drill pit removed; therefore, it appears operator has addressed outstanding pit violation. According to ES plan onsite, operator had plans to dispose of pit "onsite according to regulations." Department plans to contact operator to determine which method of disposal was utilized. Site not yet restored to final grade nor has site been seeded/mulched. Bmp's installed, Operator should continue to closely monitor and maintain all bmp's until final permanent restoration is achieved. Minor ponding of water located along access road near well site. Operator shall continue to properly grade well site in order to sustain adequate storm water drainage. Opr written response recieved 2/10/12
