1) Site sign and E & S plan were on site. E&S BMPs were in place and working adequately. 2) Site rough graded. 3) Well shut in, no production equipment installed.

1822631 2009-08-13 De minimus Violations Noted Site inspected with OGI Bruce Jankura. Well is complete but not yet set up for production. Pit has been backfilled and site recontoured with small section of cut side still remaining. Seed and Mulch has been placed to edge of pad leaving working area at site. There is also an area about 150 ft x 50 ft across the road from well that appears to be used for equipment storage. There is not a tag in place and site ID sign about 1/4 mile down the road making it difficult to identify well. There does not appear to be any E&S control issues at this time.

Large pool of water on the SW side of the location. Production water tanks were delivered while we were on site. Site ID sign post was located before the location on the access road, but the actual Site ID sign was not present.

-Well is complete and site was recently used to stage frac tanks. All but a few of the tanks have been removed -a Flat bed truck was on site dropping production tanks. -on back edge of pad, just below remaining cut, a pond about 6" deep and approximately 10' in diameter has formed, it appears to be fresh water from spring seeps and or snow melt. The water is emptying into the diversion ditch and being carried off-site. -I contacted Nathan Wells via phone on 06Jan10 concerning the pond. He informed me that some site-leveling and recontouring at this well has been planned.

Access road is stable. No signs of accelerated erosion/sedimentation on access road or road entrance. Site appears to be stable. No signs of accelerated erosion/sedimentation on site. Pit appears to be sound, and had adequate freeboard. Remarks (Continued):

  • Although BMP's are in place in accordance with the E&S plan, some minor maintenance is required on the silt fence and hay bales at
     the end of the cut diversion on the north corner of the site (down-slope corner of site).  There is also a small amount of an oily substance 
     at this same corner of the site that needs to be properly disposed of.
    • These concerns were discussed with operator representative Nathan Wells.
