0312890608 2013-04-01 Plug / Plug Back No Violations At the time of the inspection Jet Wire Line Services were on site for a cement bond log on the 5.5 casing. The top of the cement from the bottom plug set on 3-22-13 was tagged at 2513. The bond log showed good cement behind the casing at 2290 and some partial cement up to 2200. The casing was then shot by Jet Wire Line at 2127 and removed from the well.

Pulled the tubing and packer. Cleaned the drill mud from the well. Put a 40,000lb shear ring on the packer and ran it back into the hole. Put in 500gal of acid. Unseat the packer and put in 1000gal of fresh water and inhibitor, reset the packer at the same depth and pressure up on the annulus to 1000psi and held for 15 min. test. Lost 10 psi for a final pressure of 990 psi. Past test.

1351777087 2012-10-29 Salt Water Injection Well No Violations At the time of my inspection the pressure on the annular space between the 5.5 casing and the 2 3/8 tubing was pressured up to 140 psi. In five minutes the pressure was at 138 and at 15 minutes it was at 136 psi. This represents a 2.8% loss over 15 minutes. The annulus was pressured to 250 psi at 8 am and my test was done at noon on the same day. The drop from 250 psi to 140 psi over a four hour period represents a 3% loss. The operator of the well believes that the 5.5 longstring casing was not connected tightly and the annulus is losing pressure to formations behind the casing that are not cemented. At the time of my inspection the well was shut in due to a leak in the flow line from the pump to the well but during normal operation pressures in the tubing exceed 800 psi with no sign of lost pressure into the annulus.

I contacted the operator who indicated the well is shut in due to a leak in the line from the pump to the well. The operator will set up a time to test and service the well and will contact the necessary inspectors prior to doing so.
2037892039 2013-04-03 Plug / Plug Back No Violations At the time of the inspection a plug was set across the Trenton from 1300 to 900 with 80 sacks of 50/50 pozmix from PetroSet Cementing Services. The cement was pumped through 1.5 tubing and no circulation was observed. The cement will be tagged with the bailer after 24 hours to determine the top of the plug.
