Filter fence was installed down gradient from well pad and functioning along with a brush/grub line. It is recommended that the operator spread the grub line across the perimeter of the installed filter fence. No accelerated E&S issues were noted with access road or well site. Access road was adequately rocked and maintained. Pit sustained 2ft freeboard.
1852047 2009-12-22 No Violations Noted According to records, well permit granted on 9/23/09 to depth of 5500 ft. Spud called for 12/21/09. Well sign and E&S plan present at the entrance of site pad. Site pad was completed. E&S controls were implemented. Drilling company set conductor at 41.6¿ (13 3/8¿) sanded. First Stringer was going to 1519¿ (7¿). Current depth was at 255 ft. Water recorded on log at 145 ft. Pit aligned with 20 mil and no freeboard issues. At the time of inspection, no E&S or IW issues
1905479 2010-08-11 No Violations Noted At the time of inspection well was drilled, in production, and properly tagged. According to records, drilling completed 12/29/09. (9 month restoration deadline 9/29/10). Well site was adequately and uniformly restored with minimum of 70% successful vegetative growth to ensure permanent stabilization. No accelerated erosion or sedimentation issues. Access road stable. No IW issues.

Noted: Rock cut pile located above well site, along access road appears to be actively being utilize, unknown if operator or landowner.
