1812029 2009-07-10 No Violations Noted E&S plan and site plan posted at front of access road. Met John Blebians at field trailer. Reviewed progress of drilling horizontal well. Running at 50 ft/hr. Patterson Rig 163. Current depth at 5,372 ft below grade at 11 % degree drilling vector. 11 3/4" pipe installed to 508 ft. with 150 sacks lead cement and followed by 150 sacks tail and 20 barrels circulated. 8 5/8" to 3490 ft with 710 sacks lead followed by 140 sacks tail with 90 barrels circulated. Inspected pit. No freeboard issues and pit is aligned with plastic. Small trench installed around drilling equipment to collect and releases. Slight droplets of oil located near storage bends. Collected in trench which was draining to pit.

The cleanup process was discussed with the safety officer, Sid Manning. The spill was contained and cleaned up using soap and pads. In addition a trench was utilized to contain the fluids within an area. There were no signs of residuals at the site. Well site has active drilling on location at the time of inspection. There were no signs of erosion and sedimentation on the well pad and access road. This is a Horizontal Marcellus well. 567065

1821097 2009-08-13 No Violations Noted A CEI inspection was performed to evaluate the area in which a spill had occurred on 7/7/09. The incident was reported to the DEP. The spill was contained and cleaned up immediately using pads. There were no signs of residuals on the site at the time of the inspection. No signs of erosion and sedimentation at the well site. Pit maintaining freeboard. A large freshwater pit was being constructed at the time of the inspection.

Small trench installed around drilling equipment to collect and any releases. Samson in the process of completing frac pit located on the eastern portion of the site.
1837086 2009-10-08 No Violations Noted At the time of inspection, the well was drilled and prepared for hydro fracturing. The well was properly signed with E&S plan onsite. The access road was in good condition. No accelerated E&S issues were noted in relation to well site or access road. Pits sustained 2 ft of freeboard. It is recommended that pit freeboard continued to be monitored closely as areas of the pit were approaching the 2ft limitation.
1889551 2010-06-02 No Violations Noted At the time of inspection, well was drilled and in production. Site was properly signed with E&S plan onsite. Drilling was completed on August 20, 2009. Final restoration activities were underway during inspection. Pits were removed and backfilled, mulch and seed were being added at time of inspection. Well pad was rocked. I was informed by contractor, that farmer plans to utilize fields to perimeter around well pad, which will be seeded and mulched approximately 25-50ft from edge of rock pad. No accelerated E&S issues were noted in relation to well site or access road. Noted that two tanks were onsite and appeared sound. Dept requested that a container of methanol be removed from direct sunlight near pump station. A follow up inspection will be conducted to ensure final restoration activities have been completed and successful growth is achieved as well as removal of methanol barrel. Note: Operator noted that weather was a factor in timing of restoration activities.
1927162 2010-11-10 No Violations Noted At the time of inspection, well was drilled and in production. Site was properly signed with E&S plan onsite. Drilling was completed on August 20, 2009.

Site adequately and permanently restored with both rock and vegetation. All pits removed. All temporary bmps removed. No accelerated erosion or sedimentation issues noted in relation to site or access road. Brine tanks sound.
