0672708173 2015-01-13 Casing String Witnessed No Violations On arrival on location I found the installing 13 3/8 inch casing at a depth of, 875 feet. Schlumberger will cement. Established circulation and had good cement returns to surface. At 95 bbls of displacement we lost circulation. Circulation was not re-established. We found the cement top at 50 feet. Will shut down for 2 hours and the grout back to surface.
1125131723 2015-06-18 Preliminary Restoration No Violations On the day of my inspection I found that the 1 H well was in production and the well 3-H was not. Both wells have been completed and put into production but due to the outlet pipeline size, only 1 well can be produced at a time. There are three propane stock tanks, three 400 bbl produced water tanks, four 400 bbl condensate tanks, that are in containment. All are properly labeled. Vegetation is well established around the well pad. The area is fenced in and secured. The site passed preliminary restoration according to ORC 1509.072 (A).
