Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Pit ligned with 30 mil plastic and security fence. No freeboard issue. Falcon was contracted. Stratigraphy consisted of coal, ss and sh. Conductor pipe set at 42¿ (13 3/8¿ size). 1st stringer (9 5/8¿) set at 230 ft. Mine shaft set at 195-200 ft. Basket set at 126 ft. Cement job used 115 sks (75 sks below basket and 40 sks on basket). Water located at 195. Anticipated depth to 4900 ft.

1950751 2011-02-17 No Violations Noted At the time of inspection, site Id sign and ES present. Well actively being drilled. Access road adequately rocked. Temporary bmps installed down gradient of pad, operator should continue to maintain until final restoration has been achieved. Note: Filter fence should be installed below brush/grub filter line. One below grade lined pit located on site, contains adequate lined freeboard. Operator plans to dispose of drill cuttings by land farming, SGWT >60in. No accelerated erosion or sedimentation issues noted. Accompanied by WQS Marc Maestra.
1953860 2011-02-24 No Violations Noted Inspection result of 24hr complaint notification from operator on 2/18/11, FWD to myself on 2/22/11.

Complaint # 277464, later referred to Geologist, Mike Morgart 2/28/11, due to alleged water loss related to this well. At the time of inspection, ID sign and Es plan present. Mud observed on township roadway, Larry Mccombs of North East Energy stated he would remove with shovel, which is actively occurring, then call for broom truck. One below grade pit onsite has been relined over top of existing liner, in order to hold fresh water for frac operation. Contractor rep stated, all liquid was removed from pit and cuttings were ¿smoothed¿ out prior to re-lining to ensure a structurally stable sub base was achieved. Operator plans to land farm cuttings, reminder that cuttings must be sampled and meet the requirements of 25 Pa. Code 78.62(b) prior to land farming. No ES or IW issues. Access rd ok. Operator should contine to monitor/maintain township rd to reduce mud due to recent thaw ,as well as all bmp's until final, permaent restoration is complete.

1954068 2011-03-02 No Violations Noted Id sign and e&s plan onsite. Drilling and frac complete. Water trucks hauling frac water off site. Street sweeper onsite. Well not yet equipped for production.
