UDI Rig 59 was actively drilling. Drainage pipe mentioned in previous inspection was still flowing fresh water. No further erosion. Red staining was still apparent but did not seem larger and was mostly dry. No violations noted at this time.

2003200 2011-09-08 No Violations Noted At the time of inspection, Well ID Sign, ES plan, and ESCGP-1 present. Access road entrance and roadway appear adequately rocked and stable. Well drilled and actively being vented while onsite, not yet in production. Temporary BMPs installed according to plan (both 12in and 18in compost soxs) and functioning. Noted ¿stressed¿ vegetation alongside and surrounding sediment traps. It appears as possible evidence that sediment traps are not properly functioning and/or water is improperly draining to traps. Although no (active) accelerated erosion or sedimentation issues are noted during this inspection, due to recent heavy rains, it is recommend that operator examine and monitor sediment traps and areas surrounding to ensure that proper drainage is occurring in order to prevent a possible pollution event. Noted red staining at toe slope of pad, again previously attributed to decomposition of mulch/tress. Accompanied by WQS Marc Maestra during inspection.
2016742 2011-11-03 No Violations Noted At the time of inspection, well sign (multi- well pad), ES Plan, ESCGP-1, PPC Plan present. Site gated and locked, as well as monitored by security. Access road and well pad adequately rocked and maintained. Well drilled. Well head protected from third party access with metal fencing. No accelerated erosion or sedimentation issues noted during inspection. Bmp's are installed and appear to be functioning, although operator should continue to monitor and maintain all bmp's until final permanent restoration is complete. Previously disturbed areas have been temporarily seeded/mulched with observed growth. No violations noted during this inspection.
1985992 2011-06-28 No Violations Noted At the time of inspection, no E&S or IW discharge issues at the site. Chief contracted UDI. 9 5/8¿ pipe set at 3800 feet with Universal performed cement using gas block. Chevron present performing site inspection. Deviation of hole is less than 1 %. DEP performed a site walk and did not find any issues.

UDI Rig 59 was actively drilling. McCutcheon was cleaning fresh water tanks and drilling mud tanks. On the north toe of slope of the pad a small diameter pipe was draining what appeared to be fresh water from underneath the site. Some on site erosion had taken place along this outfall. No sediment had left the site. Also, some red staining and standing water was evident along the rock channel after the outfall of the sediment trap. Construction was being conducted on the adjacent fresh water storage impoundment. No violations noted at this time.

Well was drilled and in production. Pad had been rerocked and graded. Freash water impoundment was stable and nearly full of water. Slopes and ditches were well vegetated both on pad and impoundment. No E&S issues. No violations noted at this time.

2067333 2012-05-04 No Violations Noted At the time of inspection, no E&S issues were present at the site. Well head was modified in order to drill gas well Grace 2H.
2069203 2012-05-10 No Violations Noted At the time of inspection the site appeared to be in sound condition. The E&S plan was onsite and properly installed and maintained. The Department notes that a significant precipitation event occurred in the past 24 hours and the site shows no accelerated erosion on the well pad area or the access road.

2107614 2012-10-09 No Violations Noted At the time of inspection the well appeared to be drilled but not equipped for production. The Erosion and Sedimentation plan was onsite and appeared to installed as outlined in the plan. No observation of accelerated erosion or sedimentation in relation to the well pad area or access road. The Department suggest the Operator continue to monitor the site weekly and after significant precipitation event to ensure the stability of the site is maintained.
