| An inspection of cleanout activity for plugging and abandonment was conducted today.  The contractor, Mr. Tony Kryder drilled through the first bridge at 370 and drilled approximately 30 into a second bridge.  He tripped out hole (TOH) and would obtain a total depth measurement of 840.  The drill bit would contain a gray soft/sticky clay-like material believed to be gray shale.  Ran in hole (RIH) with bailer on first run with no problem.  During the retrieval on a second run, the bailer got hung in the hole at approx 370 (first bridge location).  After approx 1 hour and with no success trying to free loose, clamps were then installed and the bailer was pulled with the draw works.  Due to the instability of wellbore (i.e. cave-in), historical problems at this depth within the shale, and possibility of losing tools downhole, I instructed the contractor to cease drilling at this time.  I will request approval to evaluate wellbore conditions and ensure isolation of Trenton formation.  No rods or tubing were encountered downhole.  A total of 2 hours (2:30 pm -4:30 pm) to include the retrieval of bailer was spent on cleanout.  Furthermore, this decision to cease drilling will be discussed with Orphan Well Program Supervisor, Gene Chini.

0001609628 2013-06-26 Plug / Plug Back No Violations I witnessed the plugging and abandonment of this well today. The well was plugged through tubing in three stages and filled from 837 to the surface with Class A cement. All plugs for each stage circulated, but the cement did not stay at the surface. The contractor for this project will return tomorrow after cement had settled and run tools in the wellbore to tag the top of cement. Please refer to plug report for further details.
0262707605 2013-06-25 Plug / Plug Back No Violations An inspection of plugging activity was conducted today to monitor status of well at a total depth of 840. 837 of tubing was placed in the wellbore and circulation was established via mud pump. Residual old oil near surface and from backside of 6 outside diameter casing/ 5.63 inside diameter casing near the surface would be displaced from the wellbore. The well was not gasing or making oil. Todays activity revealed that the Trenton was isolated and wellbore condition would allow for plugging and abandonment with cement from 837 to the surface.
0669146265 2013-07-16 Final Restoration No Violations An inspection of the Final Restoration revealed compliance with Ohio Revised Code 1509.072. The well site was situated on a location designated as pasture field. At the conclusion of the inspections conducted on Permit Numbers 60214 and 60215, the results were discussed with the landowner, Mr. Tim Sautter, at his residence. Mr. Sautter confirmed that he was satisfied with the restoration work on this property and the additional wells also plugged and abandoned but located within field crop.
1157263615 2013-06-11 Plug / Plug Back No Violations The contractor for this project moved onto this well location today to remove the 5.63 standard rig style wellhead to allow for the use of slips during the plugging and abandonment process. The contractor will install a 7 collar with a plate to allow for 4 wellhead. The well location was verified with a GPS reading.
1333571205 2012-04-04 Orphan No Violations Inspection to update GPS and well status for a potential Orphan Well Package. At the time of my inspection the well had 8conductor casing and 6 surface casing with casing head. Debris inside 6 casing about 6 feet below ground level. No rods and tubing visable at wellhead. The well is in a cow pasture and is surrounded by smaller trees and brush.
