| I met with John Jack and Dylan from Triad, as well as Jeff Jones from J & L Water Services,  to discuss their plans for re-working the facility.  Triad plans to clean out the storage tanks, then remove all equipment from inside the dike area, clean out and re-lay the plastic liner and cover it with gravel.  They will then place the tanks back inside the dike, adding a 100 bbl tank for oil/condensate.  They will rework the sump pump and collection hole to make it work, will move the pumphouse building off to the side and pour concrete, then replace the building back on top of the  concrete.  I told John he needed to put these plans in writing and submit to Tom Tomastik.  J & L will be doing most of the construction and BBU will be cleaning the tanks.  Triad is anxious to begin.

| Inspection this day found pump not running.  When looked over the facility, found brine kill area below and off to the side of the dike - looks like plastic liner has failed and brine has went beneath the dike brim, under the liner and killed vegetation in an area 100' x 25'.  Called operator and told him would need to shut down until issues are resolved - liner may have to be replaced and tanks removed from dike to do that.  Have often found water inside the dike and tanks  seem to overflow, thus the brine remains inside the dike and has travelled under the plastic.  Carper in agreement, will start by liming the spot, then determine and fix leakage.  6/11/12 - Talked to Ryan Carper - he says lease is about to run out - their deal with Triad has not progressed as promised and Triad tells them they will get in their next month to replace the liner and redo dike area.  Ryan says they need to dispose of water to hold the lease

Of 15% - when I checked pressure it was at 530# which is the maximum limit.  Carper has also cleaned out storage tanks, dike area is dry but still black from the water running over the tanks and coloring the gravel.  Dean thinks it is more black water than oil.  Will go ahead and clear NOV with the understanding that they must keep the dike area dry and continue to try and clean it up more.  Pressure is passable.

| Quarterly inspection due - today found dike area full of water and oil, pump was running, went back and checked well and it is over the permitted pressure - gauged at 620# and only 530# allowed.  Called Tom Tomastik (no answer), Mike Kavage who advised to tell them to shut well down.  Called Millard and told him to shut down, then Tom called back and said they didn't have to but had to have pressure down to continue to operator.  According to secretary at Carper, had been down waiting on pump parts for several days - they had finally come and were now pumping to get caught up - this was not usual.  Anyway, since they did not have to shut down, did not wait for Millard to come out - late Friday afternoon.  Will recheck Monday to see progress.

| I met with John Jack and Dylan from Triad, as well as Jeff Jones from J & L Water Services,  to discuss their plans for re-working the facility.  Triad plans to clean out the storage tanks, then remove all equipment from inside the dike area, clean out and re-lay the plastic liner and cover it with gravel.  They will then place the tanks back inside the dike, adding a 100 bbl tank for oil/condensate.  They will rework the sump pump and collection hole to make it work, will move the pumphouse building off to the side and pour concrete, then replace the building back on top of the  concrete.  I told John he needed to put these plans in writing and submit to Tom Tomastik.  J & L will be doing most of the construction and BBU will be cleaning the tanks.  Triad is anxious to begin.

Triad plans to clean out the storage tanks, then remove all equipment from inside the dike area, clean out and re-lay the plastic liner and cover it with gravel. They will then place the tanks back inside the dike, adding a 100 bbl tank for oil/condensate. They will rework the sump pump and collection hole to make it work, will move the pumphouse building off to the side and pour concrete, then replace the building back on top of the concrete. I told John he needed to put these plans in writing and submit to Tom Tomastik. J & L will be doing most of the construction and BBU will be cleaning the tanks. Triad is anxious to begin.

Retainer and plug provided the opportunity to circulate after casing perfd to prove well is circulating, zone can be squeezed. Well would not circulate so plans are to get a pump truck in, pressure up on the well to see if circulation can be realized, then cement backside of casing up past where hole is in 4.5 casing. If that doesnt work, will make into a producer in the Cow Run Sand.

Called operator and told him would need to shut down until issues are resolved - liner may have to be replaced and tanks removed from dike to do that. Have often found water inside the dike and tanks seem to overflow, thus the brine remains inside the dike and has travelled under the plastic. Millard present to perform test, had to fill annulus with 32 gal water - when pressured up to 200#, did not hold and gradually fell to 0 over a period of 15-20 minutes. Millard was afraid it would not hold as had tested it early in the month and had taken it up to 400-500# which he realized later had hurt its integrety. Must have small hole in casing. Pressure test on tubing while pump running was 530# - this part passes and well is in no danger of contaminating ground water or other formations. Called Tom Tomastik to see what he wanted operator to do but I believe will have to fix casing.

He said Carper has had well acidized with 1200 gal. Of 15% - when I checked pressure it was at 530# which is the maximum limit. Carper has also cleaned out storage tanks, dike area is dry but still black from the water running over the tanks and coloring the gravel. Dean thinks it is more black water than oil. Will go ahead and clear NOV with the understanding that they must keep the dike area dry and continue to try and clean it up more. Pressure is passable.

Note: Photos were taken and water samples were taken to Cambridge lab to be analyzed, Note: To properly fix dike owner will need to have tanks cleaned and removed from dike area and new liner installed after all contaminated material is removed from dike area and replaced with fresh soil and properly place identification at wellhead with complete information.

2013257317 2007-06-06 Salt Water Injection Well No Violations Completed yearly injection pressure test this day. Pressured up annulus to 100# - pump was not working correctly and couldnt get any higher. Held 100# for 15 min - tubing tested at 318# - also held. Operator says he had pressured up annulus on Monday (this is Wednesday) and when we arrived, annulus was back to 0 already - this well never holds its annulus pressure, yet feel if there was a pipe or packer failure, we would see a rapid fall. I feel this pipe good but standard test requires 200# for 15 min. Pumphouse had been steam cleaned, looks good, fair amount of crusty, old oil in dike area but no water. Pump running this day

Pump was running, would have liked to put my gauge on well, but valve to shut was not shutting. Contacted operator and asked to put a new gauge on - gauge read 700# and he was sure gauge was not reading correctly - and install a new valve so I could shut to put my own gauge on.
