| Upon inspection of this well it was found to be not capable of production and idle. The 7 inch diameter casing was swaged down to 2 inch diameter pipe. There was a 2 inch diameter valve, in the closed position, a short nipple then the pipe was disconnected. Pictures were taken to support my findings. Mountaineer State Oper LLC.  shall submit a written plugging schedule to Matthew Fulk at Division of Oil and Gas 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Mountaineer State Oper LLC.  Will be notified if the plugging schedule is approved or denied.

The 7 inch diameter casing was swaged down to 2 inch diameter pipe. There was a 2 inch diameter valve, in the closed position, a short nipple then the pipe was disconnected. Pictures were taken to support my findings. Mountaineer State Oper LLC. shall submit a written plugging schedule to Matthew Fulk at Division of Oil and Gas 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Mountaineer State Oper LLC. Will be notified if the plugging schedule is approved or denied.
