| Violations Noted

| Landowner had complained of a smell of gas. I met with Jeff Burke of Rainbow Oil & Gas at the well site. Owner was venting gas into the 100 Barrel steel tank and allowing it to be released from the manhole in the tank. Owner stated the lines had not yet been laid due to right of way issues with the adjacent landowners. He stated work laying the line would begin early next week. All gas vented to the atmosphere must be flared, with the exception of gas released by a properly functioning relief device and gas released by controlled venting for testing, blowing down and cleaning out wells. Unlawful venting shall cease immediately and releif devise or flare must be installed if venting is to continue.

| Witnessed surface cement job. 3 centralizers were ran at 64', 224', and at 352'. 417 feet of new 7" surface casing was set in 430 feet of 9 7/8" hole with 125 sacks of Class A cement with 1% gel, 100% excess used. Hole conditioned and good circulation established before cement pumped. During cement mix the pressures at the pump truck reached 500 psi and Formation cement shut down truck. Pressure was bled off the lines and the pipe was pulled a few feet out of the hole. Lines reconnected and circulation established immediately, continued on with job. The pressures reached 500 psi and the pipe began to move up out of the hole again, before the displacement was run. Again the pressure was released and the pipe pulled, now 10-15 feet out of hole. The job was restarted and cement returns were seen at the surface, before the displacement was ran, with less than 1 Bbl of returns. Pressured up for the third time and the well was shut in with casing full of cement. Company plans to cut off surface casing at ground level and drill out cement.

| Inspection of well found a 210 barrel steel tank and a separator inside a dike. The well was produced using a pumping unit, tubing and rods. The pump jack was powered by electricity. This well utilizes annular disposal and there was a small plastic steel caged tank by the well head. There was a small water pump near the tank and the wellhead that was connected to the small brine tank. This small pump was used to move water to another well for disposal. The small pump was labeled with out/in these did correspond correctly with moving water from the tank. I did call Jeff Burke who came out and met me on location and explained the pump. No violations noted at the time of this inspection.

0018251560 2013-07-16 Drill / Deepen / Reopen No Violations 21 feet of 10 drive pipe was set to hold unconsolidated surface materials. Review of drilling rules with producer.
0226985605 2013-12-18 Production Wells No Violations Follow up to -693532978 and complaint #4560. Laying 2 line today.
0315012153 2013-12-24 Production Wells No Violations Follow up to -693532978 and complaint #4560. A functioning pop-off had been installed at the separator.
0693532978 2013-12-12 Production Wells Violations Noted Landowner had complained of a smell of gas.
0695402086 2014-10-15 Production Wells No Violations Asked to check on well by Supervisor. No noted violations.
0784577963 2013-07-30 Drill / Deepen / Reopen No Violations Administered surface casing integrity pressure test. 404 of 7 surface casing held required 370 psi for 30 minutes with 20 psi loss. Test passed.
1086764085 2013-12-19 Annular Disposal No Violations Check on annular disposal. Surface casing visible, lines hooked up and watertight.
1213926962 2013-12-19 Annular Disposal No Violations Inspected the annular disposal connections with Inspector Josh Garland. Jeff Burke (well owner) was on site as well. All connections are water tight. No leaks were observed during inspection.
1231047577 2013-09-05 Completion Testing No Violations Log suite ran. Cement op on backside of 4 1/2 production casing was1373 , Berea Sandstone from 1684-1694, perforated from 1684-1688 with 10 shots.
1241571933 2013-12-30 Production Wells No Violations Follow up to -693532978 and complaint #4560. Lines laid and meter box being connected. Well is to be in production later today.
1373400870 2013-07-09 Drill / Deepen / Reopen No Violations Building location and digging pits.
1374594658 2013-07-22 Drill / Deepen / Reopen No Violations Witnessed surface cement job. 3 centralizers were ran at 64, 224, and at 352. 417 feet of new 7 surface casing was set in 430 feet of 9 7/8 hole with 125 sacks of Class A cement with 1% gel, 100% excess used.
1374603088 2013-07-23 Drill / Deepen / Reopen No Violations The cement had fallen back about a foot. It will be topped off with a sack of redi-mix. .
1375130319 2013-07-29 Drill / Deepen / Reopen No Violations Drilling cement out of 7 surface casing and equalizing fluid in hole to get ready for casing pressure test.
1375219858 2013-07-30 Annular Disposal No Violations Administered Annular Disposal surface casing integrity test. 370 psi was held on 404 of 7 surface casing for 15 minutes with 10 psi loss. Test passed. No Tanks.
1376317744 2013-08-12 Drill / Deepen / Reopen No Violations Currently drilling at 1400 feet, down below the Injun Sand.
1377008100 2013-08-20 Drill / Deepen / Reopen No Violations Currently drilling at 1800. Berea Sandstone encountered at roughly 1740. 4 1/2 casing being delivered today.
