| I was on location to the witness setting the last three plugs on the above well. After taking the wellhead apart there was no threads to screw a 4.5 collar and a pulling sub on to. Summit Well Service had to go to Baker Tools in Wooster Ohio to get a 4.5" "center spear". When they returned they pulled stretch and Appalachian shot the 4.5 at 3907'. Summit Well Svc. pulled 50.3' of 4.5" 11.6 # casing. Universal broke circulation pumped a 300'  big lime cement plug 5455'/5755' displaced with 56 barrels of fresh water. Summit pulled 89 joints of 4.5 casing and Universal pumped 250' 8.625 plug 751'/1001'and displaced with 11.5 barrels of fresh water. Summit pulled 22 joints of 4.5 casing. Universal pumped a 250' surface plug. Job completed. I will follow up with final restoration report.

| I met with Mr. Tom Sanor at the well site.  Mr. Sanor is the son of the land owner, Mr. Don Sanor (age 92).  Tom Sanor indicated that the well hadnt produced in many years. The well was formerly operated with a pump jack. The pump jack is gone.  The well head has a swage nipple and valve on top with a pressure gauge showing approximately 190 psi.  Two other valves are on the well head.  One is a two inch valve attached to the flow line going to the tank battery.  The second is a smaller valve, which ties into a gas line to the landowners house.  The valve to the flow line was closed.  The valve to the house was open, but Tom Sanor indicated his father has it shut off at his house because the well was not dependable.  There is no production equipment for operation of the well.  Since the pump jack was removed, Mr. Sanor said that the well had been swabbed periodically, but hadnt been for an estimated eight years or more.  The well has three 100 bbl storage tanks.  There is no dike surrounding the tanks.  There is no separator, and no gas meter.  The lone identification label on one of the the storage tanks was faded and unreadable.  I rapped each tank with my hand.  There appears to be fluid in each tank.  I researched the well on RBDMS, and found that well has changed ownership a number of times.  No production has been reported since 1996.  A permit to plug the well must be obtained within 30 days (02/23/13) of the date of this notice, and the well plugged within 90 days (04/24/13) of the date of this notice.

1358270608 2013-01-14 Production Wells Violations Noted I met with Mr. Tom Sanor at the well site. Mr. Sanor is the son of the land owner, Mr. Don Sanor (age 92). Tom Sanor indicated that the well hadnt produced in many years. The well was formerly operated with a pump jack. The pump jack is gone. The well head has a swage nipple and valve on top with a pressure gauge showing approximately 190 psi. Two other valves are on the well head. One is a two inch valve attached to the flow line going to the tank battery. The second is a smaller valve, which ties into a gas line to the landowners house. The valve to the flow line was closed. The valve to the house was open, but Tom Sanor indicated his father has it shut off at his house because the well was not dependable. There is no production equipment for operation of the well. Since the pump jack was removed, Mr. Sanor said that the well had been swabbed periodically, but hadnt been for an estimated eight years or more. The well has three 100 bbl storage tanks. There is no dike surrounding the tanks. There is no separator, and no gas meter. The lone identification label on one of the the storage tanks was faded and unreadable. I rapped each tank with my hand. There appears to be fluid in each tank. I researched the well on RBDMS, and found that well has changed ownership a number of times. No production has been reported since 1996. A permit to plug the well must be obtained within 30 days (02/23/13) of the date of this notice, and the well plugged within 90 days (04/24/13) of the date of this notice.
1370007974 2013-05-30 Final Restoration No Violations I performed a final restoration inspection on the above location. All stone was removed from the well location and stored in an area for the landowners use. The area was returned to a tillable field. Production equipment and flow lines and sales lines have been removed. FINAL RESTORATION IS PASSED.
1576551266 2013-04-25 Plug / Plug Back No Violations I was on location to the witness setting the last three plugs on the above well. After taking the wellhead apart there was no threads to screw a 4.5 collar and a pulling sub on to. Summit Well Service had to go to Baker Tools in Wooster Ohio to get a 4.5 center spear. When they returned they pulled stretch and Appalachian shot the 4.5 at 3907. Summit Well Svc. pulled 50.3 of 4.5 11.6 # casing. Universal broke circulation pumped a 300 big lime cement plug 5455/5755 displaced with 56 barrels of fresh water. Summit pulled 89 joints of 4.5 casing and Universal pumped 250 8.625 plug 751/1001and displaced with 11.5 barrels of fresh water. Summit pulled 22 joints of 4.5 casing. Universal pumped a 250 surface plug. Job completed. I will follow up with final restoration report.
