0237586482 2014-08-14 Administrative Inspection No Violations Spoke with Mr. Scott Gannon of Flat Rock Development LLC via phone call at 3:42 pm to discuss the preliminary restoration for this well. I informed Mr. Gannon of the erosive conditions present on the pad location and that the preliminary restoration needs to be performed as soon as possible. Mr. Gannon stated that they do not currently have plans together to drill a horizontal well on this location, and he agreed to have a crew perform the preliminary restoration by the first week of September.
0510696365 2018-09-13 Drill / Deepen / Reopen No Violations On this day, I am conducting a compliance notice follow-up inspection. I found the well below ground level and covered with a steel plate and gravel. Vegetation is established across the surface surrounding the well, but stressed vegetation was found downslope of the well, along the tree line. Fluid was tested within the area with multiple High and Low Range Quantabs with results ranging from 150 - 2,500 parts per million chlorides. Pictures were taken during the inspection. I spoke with Southwest Supervisor Eric Conrad and ODNR Hydrologist Jake Glascock about the issue. Further investigations will be conducted.

1352555297 2012-11-09 Drill / Deepen / Reopen No Violations I went out to rig to check progress - had not been out to location since rig moved in. Pits look good, location is large enough and rig is putting drill stem back in to begin drilling out from under conductor. Wanted to look at surface casing before going in hole and found it a different size than called for. Called Steve Bumgardner, company man, and left a message to explain the change, then called Steve Opritza and Mike McCormac and left messages questioning the change. Steve B. called back and said he had talked with Steve O. and he (Steve O.) had given approval verbally, he felt. They have changed surface from 13 3/8 with a minimum of 300 to 11 3/4 and plan to set 1700. While this meets permit conditions, not sure if they can set that much. After consulting new Well Construction Rules, found under (4)(f)(iii) that they can set as much as needed without drilling into potential flow zones. This depth of 1700 would be allowed - Steve O. called me back @3:30, saying the size of casing OK but no word on the depth. Dan Goins and Mike McCormac both feel they should stop at 300 so called rig and told them to stop - to set only 300. Flat Rock says it will be very hard to drill to 7000 with only 300 surface casing in the hole. Went out late 11/9/12 and witnessed cementing of surface casing - no problems. Total time spent for 2 days - 8 hrs. 11/16/12 - Talked with Steve Opritza - he does not remember specifically talking to Steve Bumgardner but says they could have set their Surface Casing to the top of the Berea (1700) with the stipulation that Intermediate Casing would have to be set and cemented thru the Berea. As that was their intention - to set Intermediate thru the Clinton/Medina, they could have set Surface to 1700 and met requirements. Permit conditions for a minimum of 300 Surface Casing would also have been met.
1352906028 2012-11-13 Drill / Deepen / Reopen No Violations I talked to Steve Bumgardner @ 11 AM and he reports rig is back to drilling and at 3200. The BOP and cement integrity was tested and charted by outside company and he has the graphs for me to pick up - all tests passed.
1353190490 2012-11-16 Drill / Deepen / Reopen No Violations I visited the rig and found they were 5250 and drilling. They predict they will run 8 5/8 casing thru the Clinton/Medina (@ 6100) and cement sometime 11/17/12 late afternoon. I told them I would not be out to witness. They report they have had a hard time drilling thru the red clays but, with the addition of special chemicals and mud, have been able to proceed. I talked with Steve Opritza, ODNR, this morning to verify what was discussed with Steve Bumgardner regarding the change in casing size and depth - he did not specifically recall any details but said they had discussed making some changes. We discussed the amount of surface casing required on the permit - a minimum of 300 - and he agreed they had met the permit requirements. I also asked him if the new well construction rules, specically under Surface Casing (f) (iii), which state that the surface casing could be run thru water zones that may include brackish or brine bearing zones. This casing string shall be set and cemented to surface before the owner drills into potential flow zones , (i.e. the Berea), would apply in this situation and he agreed that they could have run 1700 of casing in this well with the Berea at 1850. There were no water wells within 500. I picked up the graph showing the BOP/surface casing cement test that showed the surface casing passed the integrity test. Once Intermediate string is cemented, rig will take next week off and resume drilling @12/1/12.

I have reviewed Conductor program with the operator and believed he is following permit requirements. I have not discussed Surface casing requirements with them yet.

They have just performed the intermittent job and the depth was at 6240. There was no cement circulated back to surface. There was a basket at 980 ran. Talking with Steve they will run a string of 1tubing and circulate cement back to surface.

1477061233 2016-10-20 Drill / Deepen / Reopen No Violations On location with DOGRM Eng. Tech. Andy Thomas and Flat Rock Development Representative Scott Gannon to review this well location to satisfy semi-annual 2nd year temporary inactive status requirements. Upon inspection, it is observed that the location is mainly stable and not causing substantial erosion or sedimentation. A large scald area has appeared slightly down-gradient of, and lengthwise to, the closed drilling pit location. There are also several dead trees downgradient of the closed pit which may have been affected by leaching chlorides. This finding has prompted a field test of the scald area for potential presence of chlorides from the closed pit. Field testing with low and high range Hach Quantab chloride titration strips indicate chloride levels which exceed the measurement capabilities of the titration strip (max reading is 6078 ppm). It is recommended to Mr. Gannon that Flat Rock Development get in contact with a third-party environmental consulting firm who has the capability to delineate the extent of contamination and develop a remediation plan to submit to the DOGRM Technical Section for approval. The SW Region Supervisor, District Supervisor, Geologist, and Washington County Inspector are now notified of the situation and a compliance notice will be issued to Flat Rock Development.
1477321912 2016-10-24 Drill / Deepen / Reopen Violations Noted The purpose of my inspection today is to observe the reported problem about the closed pit location. During my inspection I witnessed a large steel plate over the well and caution tape surrounding the steel plate. I observed a drain pipe leading from the cellar that shall be removed. The cellar must remain liquid tight. While observing the area of the closed pit a large scald and several dead trees on the downhill side of the pit were found. This scald was measured to be 90 feet by 30 feet. The scalded area and dead trees are in result of high chlorides leaching from the closed pit. A field test was conducted on the scalded area on 10/20/2016 by ODNR Division staff using high range Hach Quantab chloride titration strips. The results from this field test exceeded the maximum readings of the Quantabs; which is 6078 parts per million. A remediation plan shall be submitted to the Divisions technical section for approval. Upon plan approval all contamination shall be removed and taken to a State EPA approved landfill.

1580948845 2014-08-13 Preliminary Restoration Violations Noted Upon inspection, it was observed that the preliminary restoration has not been performed for this well location. The north end of the well pad has developed significant erosive conditions causing deep rills and gullies to form on the slope. A large portion of the pad is void of vegetation and barren areas are allowing sediment-laden surface runoff to flow into drainage areas. Drilling pits have been closed. There are several trees down in the lease road, rendering the road impassable by vehicle. Mr. Scott Gannon of Flat Rock Development LLC has been notified of this situation via voicemail. Previous inspection reports indicate that an extension was granted for preliminary restoration on the grounds that plans be brought forth to show intent to kick-off and drill a horizontal well on this location. No such plans have been produced and/or submitted to ODNR DOGRM. Flat Rock Development LLC will have until 9/1/2014 to complete the preliminary restoration for this location.

Universal pumped 225 sacks to surface by hesitation pump method. Upon completing this job the 8.625casing was pressure tested to 528 psi. Leaked back to 491psi after 30 minutes. Test passed.

Upon inspection, it was observed that the lease road has been cleared of all fallen trees, the location has been graded, and the seed and mulch was staged on the location and will be spread this weekend. Childress and Childress Timber and Excavating was on location, and Mr. Gary Childress stated that the restoration activities will be completed by the September 1st deadline. I will re-inspect next week to see if the restoration is complete.
