|  Status check inspection found the well now in production. The well is equipped with a lower A section attached to the 13.375 diameter casing string with a riser pipe and both a manual and electronic transducer gauge on the 13.375 well head section, the gauge reads 0 pounds per square inch (psi). The upper B section of the well head has a riser pipe and pressure gauge present monitoring the casing pressure of the 9.625 diameter casing string with 50 psi present on the manual gauge. The 5.5 diameter casing string well head section shows 967 psi and the tubing well head section production flow line shows 620 psi. The well head is equipped with an emergency shut down valve that is controlled both remotely and by multiple electrical emergency shut down sites on the well pad. The well is selling through a sand trap system before entering the gas processing unit followed by a contact tower and glycol heater. The final gas sales point is a Blue Racer midstream sales point on the North side of the well pad. Also present is a production fluid facility consisting of 4 steel construction tanks of 400 barrel fluid volume, the tanks are situated inside a polymer lined steel containment dike area. The company has installed a low pressure vapor recovery system on the production equipment.

| My inspection today finds the drilling rig has pulled the drill pipe up to 5500 after encountering a fault at 15850.  They are currently attempting to get a consistent mud weight in the hole.  They will then trip to bottom and retrieve the rotary steerable tool.

0024988198 2014-01-13 Drill / Deepen / Reopen No Violations Company is drilling a second lateral from from this hole. The first one was plugged back. Total depth at the time of inspection was 7485 ft. The inclination was 32.66 degrees. The azimuth was 230.64 degrees. Spoke with company man on site.
0068469306 2013-12-20 Drill / Deepen / Reopen No Violations Depth at the time of inspection was 15891 ft. The inclination was 90.52 degrees. The azimuth was 329.83 degrees. They were having problems with a gas kick and were stabilizing the hole. Their mud weight was 12.1 pounds per gallon and they were moving up to 12.4 pounds per gallon.
0109595262 2013-11-25 Drill / Deepen / Reopen No Violations Checked the status of the drilling rig. Total depth at the time of inspection was 890 ft. Spoke with company man and rig manager on site. They were experiencing technical difficulties due to cold weather.
0137937541 2014-01-21 Drill / Deepen / Reopen No Violations Total depth at the time of inspection was 15000 ft. Drilling has ceased on this well and company is preparing to run casing, and cement it. Spoke with drilling consultant on site. They plan on running 14984 ft. of 5.5 in casing. I was accompanied by Mallory Stanton and Kenny Brown from central office.
0241026416 2013-12-10 Drill / Deepen / Reopen No Violations Picked up a copy of the bond log that was ran the night before on the 9.625 in. casing. The cement top was at 3925. I spoke with the company man on site. We discussed the details of the formation integrity test that they plan on doing. Universal was on-site and on standby.
0329113166 2014-09-17 Preliminary Restoration No Violations Upon inspection, it was observed that all disturbed land surfaces have been stabilized with a vegetative cover. All drilling equipment has been removed from the location, the lease road has been maintained, any drilling pits have been closed, and the entire location is diked. This fulfills the requirements of the preliminary restoration.
0410061521 2013-12-27 Plug / Plug Back No Violations The rig crew ran a Weatherford IPP packer with three 800 lb. shear pins. They set the packer at 8937. They tried to trip the shear tool, but were unsuccessful. Tim with Triad notified me that they are going to try a blind back off.
0420667440 2013-11-22 Drill / Deepen / Reopen No Violations Witnessed the cementing of the surface casing. No chlorides were detected in the mix water. Good circulation and good returns were observed at the surface.
0472360845 2018-07-16 Production Wells No Violations Status check inspection found the well now in production.
0915370870 2014-01-02 Plug / Plug Back No Violations My inspection today finds the company attempted to run a bridge plug. The plug became stuck at 6859. They are preparing to drill the plug out.
109595262 2013-11-25 0:00 DD No Violations 3.00
1168772851 2013-12-05 Drill / Deepen / Reopen No Violations Total depth at the time of inspection was 5971 ft. Company plans to drill to a depth of 6930 ft to set 9.625 in casing. This is the nudged out section of the vertical wellbore. Spoke with company man on site.
1388778300 2013-12-30 Plug / Plug Back No Violations My inspection today finds the packer fell off of the drill pipe while being retrived. The rig crew is running pipe to ensure the packer is below 8000 before they attempt to set another packer.
1389025008 2014-01-06 Drill / Deepen / Reopen No Violations Lateral has been plugged back. A mechanical bridge plug was set at about 7920 ft. Two cement jobs were performed after that. Cement top was established at 6523 ft. 9.625 in casing shoe is at 6826 ft. Spoke with company man on site.
1456409382 2017-12-28 Completion Testing No Violations Status check inspection on the well completion process found Stingray Pressure pumping on location completing stage 37 of 40 to be stimulated. The well bore is being completed in 150 increments and perforated by Kodiak Wireline Service with 5 clusters of 7 shots each cluster. The total sand used is 175,000 pounds per stage with a total treating dirty water total of 7,200 barrels. The initial shut in pressure is 3,000 psi. All annulis is being monitored electronically and hooked into a gas buster tank flowing through a flow valve. The well head is a Universal Well Service 15,000 psi rated working pressure along with all the pressure pumping equipment. All stimulation equipment and support equipment is situated on polymer containment.
1457370514 2016-03-07 Administrative Inspection No Violations This P# is assigned to the original lateral section of the 1306H, which was a lost hole and was plugged back.
1505303667 2014-01-23 Drill / Deepen / Reopen No Violations I arrived on site to witness the cementing of the 5.5 in production casing. Casing had been ran and company was circulating mud in the well. Due to extremely cold tempuratures pumps and lines were freezing. I wasnt able to witness the entire job. They were pumping cement very slowly when I left. Good circulation had been maintained during inspection. Spoke with drilling consultants on site.
1658310647 2018-11-14 Preliminary Restoration No Violations I am on location today to complete the preliminary restoration on this well. The preliminary restoration meets 1509.072(A). There are no violations at this time.
