| I inspected this producing well as part of routine inspection activity. At the time of this inspection, I observed the following: the wellhead pump jack system is located in a chain-link enclosure. Fittings and connections at the wellhead were secure from leakage or problems. The areas surrounding the wellhead were comprised of a narrow unpaved roadway with residential homes to the south and nursery facility to the north. I walked the area surrounding the wellhead and did not observe any signs of impact or problems. The associated tank battery is located near the main roadway and is shared with the #-2-882 and #2-0884 units. The load-lines were plugged and secure. The tank lid was closed. The secondary containment was dry and free from signs of leakage, spills or other problems. The access road to the battery and well is a shared-use road with the nursery facility and was in good condition. There were no issues or problems requiring attention at the time of this inspection. I noted that there was no apparent production report filed for the 2015 reporting period. I contacted the well owner and he indicated that he would investigate to determine where the missing information was. As of 11/04/2016, the owner had electronically transmitted the Form 10 annual production data for 2015 to ODNR in Columbus.

This well was converted from a plunger lift system to a pumping unit.  The landowner, who purchased the adjoining lot south of the well,  built his house with the sun room windows facing directly at the well. The well has been fenced with new 6' chain link.  I spoke Ron Gibson, who will install a section of 8' board on board fence to block the the view of the well from the home.  I spoke to the landowner immediately west of the well and he stated that the well is not an issue for him as his shed blocks any view of the well.  The Village of Perry does not allow buildings within 300' of a gas well.  I notified the Mayor, who asked for my opinion on the village spacing rules, that ORC 1509 requires 150' spacing for an inhabited structure to a wellhead in urban areas, unless the landowner approves a waiver for 100' and for unoccupied structures, there are no rules.
