The plugging procedure was discussed between ODNR Inspector William Stolarik and G-R Contracting Representative, Jordan Jones. G-R Contracting has a rig on the hole, (1) water-truck, and a plastic-lined earthen pit for fluid collection. Formation Cementing has a pump-truck and bulk-truck on location. The well was originally drilled to a depth of 870 feet to the Lime Formation, with 7 inch diameter casing set at a depth of 292 feet. 4.5 inch diameter casing was never ran in the hole. The well was unproductive due to cave-in that filled the hole to a depth of 560 feet. 1.9 inch diameter tubing is inserted into the hole to a depth of 560 feet. Circulation was established and Formation Cementing pumped a cement plug from 560 feet to surface that consisted of 145 sacks of Class A cement with 1.18 yield and a weight of 15.6 pounds per gallon. Then, all tubing was pulled from the hole, and the well was topped off with an additional 12 sacks of Class A cement with 1.18 yield and a weight of 15.6 pounds per gallon. Cement was holding steady at surface prior to leaving the job. | ||

There is a 7 collar with a short nipple and steel plate welded to the nipple above ground level. Attached to the top of the steel plate is a 2 diameter collar with a production valve screwed into the collar. The valve was observed to be in the closed position. There is a 2 diameter steel flow line attached to the production valve that was found severed between the well head and the nearest metering point that services 3411123229-0000. There is no related meter or production tank found associated to the said well. The original permit condition called for 4.5 production casing and the well card on file indicates that 4.5 production casing was set, however no 4.5 diameter production casing is visible at the well head indicating the original drilling permit condition was not followed. The last reported commercial production was in 2015. Instructions are as follows-The well owner/operator shall submit a written plugging schedule to Inspector Bill Stolarik at 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. | ||
