| On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072 (A) with vegetation established and no substantial erosion. The well is equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. It is setup to be produced by means of a pumping unit with rods and a gas engine. The production facility consists of (3) 100 barrel steel tanks, (1) 65 barrel steel tank, a drip tank,  domestic meter, and a vertical separator all contained within an earthen dike. The facility is shared with permit numbers 2-4299 and 2-4281. The production records are current and the tank is identified.
