| Well has been complete under the special revised permit conditions. But the completion Pipe Tally records showing both the pipe tally and placement of centralizers on the production string and the casing pressure testing charts were never given to the inspector upon His request by the previous company representative Ron Giles, The new replacement consultants and myself looked through the completion records on hand and no information was found. The inspector also addressed an issue with a pressure leak at the well head and the protection fence being not up around the well head and both issues were addressed immediately by the company men on site.

Upon further review it was discover that the casing guidelines were not being followed as the permit casing program called for and the 9.625'' string was going to be omitted. Inspector called both corporate office and the field representative ''Ron Giles'' and had drilling progress stopped and required the company to comply with the permit conditions. Also noted, both inspector Pat Shreve and my myself have asked the company to cover the open cellar or tin horn set in the ground for the other well on the pad, before someone falls in it.

There was Bond log ran on the 13.375''casing. I am not sure how the company RWS  ran their log. According to cement ticket from Universal they had proper lift pressure. Also noted that the company was not prepare to cement the casing by not having any fresh water on location and almost flowed their return tanks over while cementing the casing string.

But the completion Pipe Tally records showing both the pipe tally and placement of centralizers on the production string and the casing pressure testing charts were never given to the inspector upon His request by the previous company representative Ron Giles, The new replacement consultants and myself looked through the completion records on hand and no information was found. The inspector also addressed an issue with a pressure leak at the well head and the protection fence being not up around the well head and both issues were addressed immediately by the company men on site.

Also noted, both inspector Pat Shreve and my myself have asked the company to cover the open cellar or tin horn set in the ground for the other well on the pad, before someone falls in it.

Inspector will note that I made a phone call to Eclipse Resources field representative Melissa Hampshire regarding the soil that has been disturbed around the drill site perimeter boundary. It was agreed upon by both parties that once the drilling process was complete and the site cleared from equipment that a on site meeting would take place and corrective actions would then be initiated. The well has been drilled to the marcellus and cemented in place. than there were 6 holes shot in casing and a mini frac was performed. 20 barrel of fluid was pumped and broke down the formation. The well was being under test for measuring pressures for 14 days. Stoarick and I and Melissa and Billy measured pressure from back side from bottom to top. All were at 0 psi.
0056451807 2018-03-08 Production Wells No Violations Status check inspection found the well pad equipped with a casing and tubing well head assembly with (2) primary master valves on the tubing head assembly followed by an emergency shut down valve. The production valves are in the open position. There are multiple emergency shut down control points on location including the well pad entry point outside the gated and fenced well pad area. On location is a production facility consisting of a total of (5) production tanks of steel construction design all situated inside a polymer lined steel containment dike area. On this day I contacted the company production supervisor and discussed annular pressure monitoring.

The tool pusher will notify the County inspector upon completing BOP test. There was Bond log ran on the 13.375casing. I am not sure how the company RWS ran their log. According to cement ticket from Universal they had proper lift pressure. The company feels satisfied that the job was correct. I did not witness job. County inspector Bill Stolarick did.

0550544485 2018-06-07 Production Wells No Violations On this day, I am conducting an inspection by request of ODNR Inspector William Stolarik in reference to an air quality complaint by a landowner. No unusual smells are indicated at this location. The well isnt equipped with annular pressure monitoring devices on the surface and intermediate casing strings. The company will need to have gauges installed for proper monitoring. The facility is gated with proper signage and an electronic shutdown button at the entrance. There are (5) 400 barrel steel tanks within containment, at the front left-side within containment. (2) gas processing units are located at the back left-side. Inspector will note that the company is proposing to set 500 of 20 to be cemented back to surface as their cemented conductor. Permit called for 100 minimum conductor cemented back to surface. Inspector did not witness 24 being set.

0696134309 2013-02-21 Drill / Deepen / Reopen No Violations Status check inspection @ 11 AM to check on the running of the 5.5 production casing found the current depth to be 10,200 with the bore hole depth of 11,799 and the company circulating the casing into the bore hole. Inspector returned at 4 PM and the company was still running casing and approximately 300 short of their proposed total depth. Company man indicated they were at least 4 hours from being ready to start rigging up the cementers. I indicated to Him that I would pick up the cement long string tickets the following day.

Inspector discussed with the company man on site about having the drilling rig and equipment washed down before removal from the drill site in a area that would allow the recovery of the waste water.

0961889198 2012-09-25 Drill / Deepen / Reopen No Violations Inspector received a call at 7:30 AM from Ron Giles stating that the 13.375 casing was on bottom and they would be cementing in a couple of hours, upon arrival on location inspector found that the casing and borehole was short of the permitted TD for the casing strings intended isolation. Inspector noted to the company man that the next string which will be the 9.625 must cover the Berea per permit conditions. Also noted that the company was not prepare to cement the casing by not having any fresh water on location and almost flowed their return tanks over while cementing the casing string.
1219811434 2012-10-13 Completion Testing No Violations Well has been complete under the special revised permit conditions. But the completion Pipe Tally records showing both the pipe tally and placement of centralizers on the production string and the casing pressure testing charts were never given to the inspector upon His request by the previous company representative Ron Giles, The new replacement consultants and myself looked through the completion records on hand and no information was found. The inspector also addressed an issue with a pressure leak at the well head and the protection fence being not up around the well head and both issues were addressed immediately by the company men on site.

Inspector will note that a copy of the casing laws was given to company man on locationRon Giles.
1348498769 2012-09-24 Drill / Deepen / Reopen No Violations Status check on drilling rig, current depth of 2342 using a 8.750 bit. Drilling company was stuck in hole at the bottom of the surface casing for 2 days. Upon further review it was discover that the casing guidelines were not being followed as the permit casing program called for and the 9.625 string was going to be omitted. Inspector called both corporate office and the field representative Ron Giles and had drilling progress stopped and required the company to comply with the permit conditions. Also noted, both inspector Pat Shreve and my myself have asked the company to cover the open cellar or tin horn set in the ground for the other well on the pad, before someone falls in it.
1348530311 2012-09-24 Drill / Deepen / Reopen Violations Noted Inspector was on site and company is attempting to drill well to TD without setting 9.625 through Berea, and has altered casing program from original casing program permit conditions as issued by the division. Inspector notified both the company headquarters and the field office representative Ron Giles that any further attempts to deepen well without first setting, properly centralizing and cementing back to surface the 9.625 intermediate string through the Berea or approximately 2100 would find them in further violation. Also proper notification must be made to the Division before casing strings are run into the well bore.

Upon learning of the companys plan to cement the 5.5 casing into the well bore I asked the company man to have his intended plan confirmed through permitting.

A plug was set through 2.875 tubing inside the 5.5 casing covering the perforations from the depth of 5735 to 4700. The 5.5 will be cut and recovered at a free point above 4700.
1359466167 2013-01-29 Drill / Deepen / Reopen No Violations Inspection and status check on the setting depth of the bottom plug inside the 5.5 casing and the cutting of the 5.5 casing at the determined free point. The company man on site stated that the plug was tagged @ 4704 and the casing cut @ 4698, they are currently circulating oil based mud into the bore hole for stabilization reasons, the casing will then be pulled out of the hole and drill pipe ran in to spot the kick plug. The company man on site will contact me with an estimated time on setting the next plug.
1360277283 2013-02-07 Drill / Deepen / Reopen No Violations Up on inspection the current drilling depth was found to be 4239 using a 8.750 bit, with the current down hole position just starting to build their horizontal curve. Inspector will note that I made a phone call to Eclipse Resources field representative Melissa Hampshire regarding the soil that has been disturbed around the drill site perimeter boundary. It was agreed upon by both parties that once the drilling process was complete and the site cleared from equipment that a on site meeting would take place and corrective actions would then be initiated. Also it was observed that crews were on site maintaining erosion and run off control.
