+ | | On this day, I am accompanying ODNR Inspector William Stolarik in response to an incident that occurred. Please review his comments for a detailed report. At 2:40pm I received a call from Statoil Company Man, Colin Hecker, informing me that they heard sounds coming from a previously drilled well on the pad, so they shutdown operations, moved the crew to a muster point, contacted the Fire Department, and called the ODNR Incident Notification number as a precaution. Once onsite, we spoke with Company Man, Colin Hecker and Chris Woodall, and Completions Superintendent Darren Galisky. We found that a needle valve had been broken on the riser pipe for the 9.625 inch diameter casing due equipment traveling over steel plates that covered the existing wellheads, and depressing them into the valve. It was witnessed that the casing valve and secondary valves were not closed, allowing the pressure within the casing to bleed-off into the cellar. | ||

The rig has completed the drilling of the 12.250 intermediated well bore- ran 9.625 diameter casing and cemented the casing. A machine used in the drilling support process was traveling across the steel plates placed across the previous drilled wells when a needle valve connected to a riser pipe that was attached the 9.625 diameter casing well head section began leaking. The casing valve was found in the open position. The steel plate was removed and the casing valve closed to remedy the situation. | ||
