| On this day, I found the hydraulic fracturing process has been completed, and the crew is currently demobilizing and leaving the pad. I spoke with Equinor Company Man Tyler Sanregret during the inspection. He stated that Deep Well will be rigging up a standalone snubbing unit to drill out the frac plugs, commencing the week of 5/20/19. | | On this day, I found the hydraulic fracturing process has been completed, and the crew is currently demobilizing and leaving the pad. 0395994163 | 2018-07-16 | Production Wells | No Violations | On this day, I am accompanying ODNR Inspector William Stolarik in response to an incident that occurred. Please review his comments for a detailed report. At 2:40pm I received a call from Statoil Company Man, Colin Hecker, informing me that they heard sounds coming from a previously drilled well on the pad, so they shutdown operations, moved the crew to a muster point, contacted the Fire Department, and called the ODNR Incident Notification number as a precaution. Once onsite, we spoke with Company Man, Colin Hecker and Chris Woodall, and Completions Superintendent Darren Galisky. We found that a needle valve had been broken on the riser pipe for the 9.625 inch diameter casing due equipment traveling over steel plates that covered the existing wellheads, and depressing them into the valve. It was witnessed that the casing valve and secondary valves were not closed, allowing the pressure within the casing to bleed-off into the cellar. |
1277948208 | 2018-07-16 | Casing String Witnessed | No Violations | On site inspection as a response to a reported release of pressure from the said well. The well has previously been reported to of been isolated as a simultaneous operation permit condition to allow the company to drill additional wells on the well pad. My investigation found that Neighbors Drilling rig X13 was on site and currently drilling the well identified as 3411124826-0000 The Eisenbarth SU#1-H. The rig has completed the drilling of the 12.250 intermediated well bore- ran 9.625 diameter casing and cemented the casing. A machine used in the drilling support process was traveling across the steel plates placed across the previous drilled wells when a needle valve connected to a riser pipe that was attached the 9.625 diameter casing well head section began leaking. The casing valve was found in the open position. The steel plate was removed and the casing valve closed to remedy the situation. |
1300302518 | 2014-02-11 | Drill / Deepen / Reopen | No Violations | Status check inspection on the drilling progress found the current drilling depth to be 4449 using a 8.5 drill bit with the projected kick off point to be 6000. |
1384796306 | 2013-11-18 | Drill / Deepen / Reopen | No Violations | The said well was spudded by on 11/13/2013 by Butches Rat Hole with 100 of 20 conductor being set and cemented to surface, A 20 diameter mouse hole is being drilled with a 16 pipe to be set and cemented by grouting to surface with a .500 thick steel plate on bottom. |
1392848996 | 2014-02-19 | Drill / Deepen / Reopen | No Violations | Status check inspection found the well drilling process has been completed and the 13,409 of 5.5 20# production casing has been ran and cemented with a calculated top of cement of 100 from surface, the drilling rig is skidding to 34-111-2-4420-0000. |
1520794206 | 2014-02-09 | Drill / Deepen / Reopen | No Violations | Notification was received by the company man on site that they will be cementing the 13.375 1st intermediate drill string through the night. |
1557932082 | 2019-05-15 | Completion Testing | No Violations | On this day, I found the hydraulic fracturing process has been completed, and the crew is currently demobilizing and leaving the pad. I spoke with Equinor Company Man Tyler Sanregret during the inspection. He stated that Deep Well will be rigging up a standalone snubbing unit to drill out the frac plugs, commencing the week of 5/20/19. |
1609563017 | 2020-5-18 0:00: | PW | No Violations | 0.30 |
2010751777 | 2013-12-24 | Drill / Deepen / Reopen | No Violations | On 12/24/13 Orion drilling is now on rigged up on the hole and the 17.5 surface bore hole is drilled to 530 with 517 of 13.375 surface casing set and cemented to surface by Halliburton cement services with 10 barrels of cement returns to surface, a sing in cement latch down collar was used on the surface cement job. Inspector did not witness due to the Christmas holiday. |
