0059858161 2016-11-04 Casing String Witnessed No Violations The purpose of my inspection today is to witness the 120 feet deep 30 inch diameter conductor being cemented into a 36 inch diameter hole. No water was encountered during the drilling process. Stingray Cementing is on location to complete the cement operation. During my inspection Stingray pumped 15 barrels of gel, 10 barrels of water ahead of 82 barrels of Class A cement at 15.6 pounds per gallon and displaced with 87 barrels of water. 25 barrels of cement returns were achieved.
1402361644 2016-12-20 Casing String Witnessed No Violations The purpose of my inspection is to witness the cementing of the 18.625 inch diameter surface/mine string casing into a 24 inch diameter borehole. During my inspection the HP Rig 531, after many attempts, could only get the casing down to 771 feet and could not go any further. The plans now are to lay down the casing and clean out the hole. Cement job will be delayed one more day.
1482829644 2016-12-22 Casing String Witnessed No Violations The purpose of my inspection today is to witness the surface-mine casing string cement job. HP Drilling Rig 531 drilled a 26 inch diameter borehole to a depth of 910 feet. The mine void is from 782 feet to 800 feet in depth. The 18.625 inch diameter casing was run to a depth of 851 feet. Schlumberger is on location to cement this casing string in two stages with a packer set at 604 feet. 300 barrels of fresh water and 20 barrels of gel were pumped ahead of the cement. 83 barrels of Class A cement with 1.5 percent calcium and 234 barrels of displacement were pumped to complete the first stage. The plug landed and the packer was energized. The second stage was completed using 273 barrels of Class A cement with 1.5 percent calcium pumped after circulation to surface was established. 186 barrels of displacement were pumped and circulation was lost temporarily then reestablished. Water returns were established but cement returns did not reach surface. Schlumberger preformed a top off with Class A cement weighing 15.6 pounds per gallon and cement was brought to the base of the cellar.
2145177009 2017-02-23 Completion Testing No Violations At 10:33, I received a call from Eclipse EHS Supervisor Bruce Carpenter who stated that during the frac job 30 barrels of 28% Hydrochloric acid was released onto containment from the belly of a third party truck at 10:25 and Halliburton has neutralized the acid with soda ash and was in the process of cleaning up the residue. Upon arrival at 12:15, I met with Eclipse Consultants Dustin Moore and Mike Hartshorn who stated that only 380 gallons was released onto containment. S3 pumping had stated that all 1257 gallons was released but after strapping the tanker it was revealed that 877 gallons remained inside. Mr. Moore stated that Halliburton went to pull the tractor out from underneath the tanker transport and they unhooked the airlines which deflated the rear airbags forcing a bind on the jack stand. When they moved the tractor the fiberglass line between the manual valve and the air actuated safety valve broke releasing the acid. The air actuated safety valve was shut. The tanker which set on containment was pulled onto a second containment so that the acid could be neutralized with soda ash. Crews onsite began flushing the soda ash and vacuuming it up for disposal. Upon completion of flushing the containment will be pulled up and disposed of and replaced. This will reveal frac primary containment. If frac primary containment is damaged it will also be replaced. The remaining acid on the tanker will be transferred to another tanker prior to the tanker being removed from the location. All acid released was within containment.
