0749585449 2016-12-28 Casing String Witnessed No Violations I was on location to witness Express Energy cement the 30 inch diameter conductor casing into a 36 inch diameter bore hole. Express drilled a 100 foot 36 inch diameter bore hole and set 100 inches of 30 inch diameter conductor casing into the hole. Express ran 13 barrels of fresh water gel ahead of the cement. Express then ran 275 sacks of cement, 2% CaCL, and .50 pounds per sack of floseal with 57.79 barrels of fresh water at 15.6 pounds and a yield of 1.18. Express then circulated with 74.80 barrels of fresh water until we got cement to surface. Express circulated about 10 barrels of cement to surface and finished cementing the conductor into the bore hole.
1296439385 2019-04-04 Production Wells No Violations On this day, I am accompanied by ODNR Inspector Rick Brown. I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units, (2) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 500 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 25 psi. Production records are current.
1721578600 2017-03-13 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Well Service Group rig # 1 on the hole. The rig has drilled a 26 surface borehole to a measured depth of 470 and ran 432 of 20 diameter 94 lb/ft J-55 buttress thread casing with a double valve sting in float shoe ran and proper centralizer placement demonstrated. Halliburton Cement service cemented the casing using 155 barrels of NeoCem Class A cement consisting of 705 sacks of cement. The cement was pumped through the drill string stung into the double valve float shoe.15 barrels of cement returns were witnessed to surface. The floats did not hold in the casing shoe.
1871675671 2017-12-14 Completion Testing No Violations Status check inspection on the well completion process found the well drilling -stimulation and completion process has been completed and crews are on site installing the production well head assembly. The location is also being cleaned up to be prepared to turned over to production.
1983122949 2018-03-06 Completion Testing No Violations Status check inspection found the well drilling and completion process now complete. There is a primary master valve installed on the well head. The well is set up to be produced out of the 5.5 production casing with a production stack assembled on the casing well head consisting of (2) manually controlled valves and (2) emergency shut don valves. Crews are on site installing the production equipment and flow lines. A production fluid facility is being assembled and will consist of a total of (2) steel tanks capable of holding 400 barrels total capacity each. The wells are plumed up to accommodate annular pressure monitoring. The preliminary restoration will need followed up on later in the year once vegetation can be better established. The well pad outward slopes appear to be stable and the well pad surface is of good repair and of stone construction.
