1056993270 2017-09-26 Drill / Deepen / Reopen No Violations On the day of my inspection I found the H&P FlexRig 521 has drilled to a depth of 14,312 feet MD . They are shut in and not currently circulating or flaring. Gas units are at 37 with 263 pounds per square inch on the casing and 226 pounds per square inch on the drill pipe. Mud weight is currently 14.8 pounds with a plan to increase to 16.2 pounds to kill the well from surface.
1500993846 2017-07-25 Casing String Witnessed No Violations The said well was spudded on 07/24/2017 by HAD Drilling rig 618 with a 22.250 conductor well bore drilled to 119 and ran 119 of 20 diameter .375 wall weld connection casing. Fresh water was encountered at 30. O-Tex cement service was on location to cement the casing using a 33 barrel cement mix consisting of 155 sacks of class A cement with 2% calcium 0.25% cello-flake 0.2% X-air additives. The cement weight was verified by weight of scale at 15.6 pound per gallon and displaced to 110 with 39 barrels of fresh water. Cement returns were witnessed to surface.
1506347758 2017-09-25 Drill / Deepen / Reopen No Violations On the day of my inspection, I accompanied ODNR Inspector William Stolarik. Please reference his comments for a detailed report. Upon inspection I found that the H&P FlexRig 521 has halted drilling at a depth of 14,312 feet MD. The most recent survey shows an azimuth of 330.5 degrees and an inclination of 91.2 digress at 14,249 feet MD. They are currently in the 8.5 inch diameter wellbore and took a gas kick which prompted them to call Boots and Coots on location to monitor the situation, along with contacting the local fire department. They are currently flaring with 1,741 units of gas and 769 pounds per square inch annular pressure. This was the third kick in a series since the curve landed at 10,182 feet MD, and another kick at 12,900 feet MD. They suspect a fault is the probable cause of the kicks. They closed the rams early Saturday morning September 23, 2017.

1506626377 2017-09-28 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling progress found H&P Drilling rig 521 on location. The rig has drilled and set casing down to 9.625 casing string point and drilled a 8.5 production borehole to a measured depth of 14,312 in which at point the company experienced a gas influx and shut the blow out preventer pipe rams and stopped circulation. In the proceeding days multiple fluid sweeps were pumped down the well bore in excess of 1,171 barrels with the last attempt being a 500 barrel fluid pill weighing 16.1 pound per gallon. The current surface pressure on the well head area monitoring the well bore pressure on the backside of the 5 drill string is 142 psi. The company believes the well bore is not yet to a static level safe enough to open the pipe rams and attempt to begin to pull the drill string from the well bore and is going to increase their mud weight to 17.5 pounds per gallon and attempt to pump another annular kill sweep.
