| Landowner, Larry Cole, called to see if I knew who owned well - says it is leaking considerable gas and feels a safety issue.  Believes well is on his property.  I was not sure when I had inspected prior but wondered if it belonged to church - well is right behind church and someone was tied into well for gas.  NO ID'S!  Larry says church uses gas - after much discussion, felt Susie Rinard might own well.  Larry gave her a call, then called me back saying she does own the well and would send somebody out to look at it.  Explained to him that she was in non-compliance due to no bond/insurance and no ID's.  Larry wanted to file complaint - I have asked Susie at least 2 times to become registered operator and still not done so will now write violation.  I will get out to address gas leak at first available time.  If she had ID's up with emergency contacts, would not have had to contact Columbus or inspector.  To resolve, must become bonded and insured, add all wells to the bond, report annual production, and post ID's at all wells with ownership information, permit numbers and 2 emergency contact numbers.

Owner has gained compliance as far as environmental regulations.  However the owner still needs to register with the state.  I spoke with Mr. Bobby Kroll.  His wife handle's the paperwork for Ms. Susan Rinard.  He stated he would ask her about it.

| Inspected well this day, due to complaint.  Several close landowners say this well on church property, it is getting gas, but Larry Cole said it was on his property, leaking gas,  and that Susan Rinard had paid him royalties, which would lead you to the conclusion that well is on his property.  Well has jack, very faded IDs from probably 1980s but no owner or permit no or phone numbers - old tank and a new tank, no dike.  Ditch cut from tank downhill into driveway of church - altho it is Exempt Miss, ditch should not be there - not really a kill area from the water but needs to be more discreet when disposing of the water.  Operator told Larrey Martin, who services her wells, says that she claims she is bonded and insured - needs to talk with me and inform Columbus of this information.  Will get all her permit no. together and sent to her.  Very slight odor of gas - not of a concern to me at this time.

| On March 11, 2014, I accompanied Inspector Andy Thomas on a follow up inspection of this well. Upon inspection, it was observed that this well is not yet in compliance. The brine line coming out of the knockout tank is still draining into the dike, which overflows onto the surrounding ground creating a scalded area approximately fifty feet long by as much as fifteen feet wide. Quantabs were used to test chloride levels of the released brine water at the source pipe and midway down the scald zone. The source test yielded a chloride level of 6078 parts per million (ppm) which is the maximum readable level for the Quantab test strip. The scald zone sample yielded a chloride level of 1082 ppm. Andy Thomas will contact the well owner (Susan Rinard) and notify her that brine may not be released in this manner, and the scalded area must be remediated.

| Owner has not made any attempt to gain compliance.  The brine line from the knockout tank still drains into the dike and the dike overflows and brine water is released onto the ground in an area 50 ft long by as much as 15 ft wide creating a scalded area.  A Quantab test was administered in the scald area in a small puddle. The result of that test was a reading of 5.0 which equals 1082 parts per million (ppm) chlorides.  Another Quantab test was administered at the opening of the brine line.  The result was that it maxed the Quantab out which means that the chloride content is greater than 6078 ppm.  I will contact the well owner and tell her once again that brine cannot be released on the ground and scald must be remediated.  New pictures were taken.  Accompanied on inspection by Inspector LJ Parkins.

| Inspected well this day, due to complaint.  Several close landowners say this well on church property, it is getting gas, but Larry Cole said it was on his property, leaking gas,  and that Susan Rinard had paid him royalties, which would lead you to the conclusion that well is on his property.  Well has jack, very faded ID's from probably 1980's but no owner or permit no or phone numbers - old tank and a new tank, no dike.  Ditch cut from tank downhill into driveway of church - altho it is Exempt Miss, ditch should not be there - not really a kill area from the water but needs to be more discreet when disposing of the water.  Operator told Larrey Martin, who services her wells, says that she claims she is bonded and insured - needs to talk with me and inform Columbus of this information.  Will get all her permit no. together and sent to her.  Very slight odor of gas - not of a concern to me at this time.

| Owner has not made any attempt to gain compliance.  The brine line from the knockout tank still drains into the dike and the dike overflows and brine water is released onto the ground in an area 50 ft long by as much as 15 ft wide creating a scalded area.  A Quantab test was administered in the scald area in a small puddle. The result of that test was a reading of 5.0 which equals 1082 parts per million (ppm) chlorides.  Another Quantab test was administered at the opening of the brine line.  The result was that it maxed the Quantab out which means that the chloride content is greater than 6078 ppm.  I will contact the well owner and tell her once again that brine cannot be released on the ground and scald must be remediated.  New pictures were taken.  Accompanied on inspection by Inspector LJ Parkins.

Contaminated area outside of the dike has been remediated. Owner has gained compliance as far as environmental regulations. However the owner still needs to register with the state. I spoke with Mr. Bobby Kroll. His wife handles the paperwork for Ms. Susan Rinard. He stated he would ask her about it.

0474744511 2014-03-11 Production Wells No Violations On March 11, 2014, I accompanied Inspector Andy Thomas on a follow up inspection of this well. Upon inspection, it was observed that this well is not yet in compliance. The brine line coming out of the knockout tank is still draining into the dike, which overflows onto the surrounding ground creating a scalded area approximately fifty feet long by as much as fifteen feet wide. Quantabs were used to test chloride levels of the released brine water at the source pipe and midway down the scald zone. The source test yielded a chloride level of 6078 parts per million (ppm) which is the maximum readable level for the Quantab test strip. The scald zone sample yielded a chloride level of 1082 ppm. Andy Thomas will contact the well owner (Susan Rinard) and notify her that brine may not be released in this manner, and the scalded area must be remediated.

Larry says church uses gas - after much discussion, felt Susie Rinard might own well. Larry gave her a call, then called me back saying she does own the well and would send somebody out to look at it. Explained to him that she was in non-compliance due to no bond/insurance and no IDs. Larry wanted to file complaint - I have asked Susie at least 2 times to become registered operator and still not done so will now write violation. I will get out to address gas leak at first available time. If she had IDs up with emergency contacts, would not have had to contact Columbus or inspector. To resolve, must become bonded and insured, add all wells to the bond, report annual production, and post IDs at all wells with ownership information, permit numbers and 2 emergency contact numbers.

1080103790 2013-11-18 Production Wells Violations Noted Well has been pumped recently. It is equipped with a pumping unit and an electric motor. There is one 100 barrel tank in the tank battery with a knock out on top of it. Brine has been draining from the knock out into the containment dike via an open ended pipe in the knock out. It had breached the dike wall and was running down the access trail and caused vegetation kill. I spoke with Mrs. Rinard, who owns the well and told her that she wasnt allowed to do this. She agreed to my terms and will be making provisions to have brine hauled. We spoke via cell phone on 11/19/13 at 8:42 AM.
