| I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked.  Wells with permit numbers 34067212200000 and 34067211080000 are operated by a plunger lift system. Permitted well number 3406721218000 is a reservoir pressure driven well. RBDMS production records indicate that the wells are in production.

Schlumberger Cementing Service c/w 554 sks of class A cement. Was advised at 6 am that they would be done cementing at 11 am. I found them on schedule and the casing would not make up and job ran over.

0448697236 2016-02-16 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. Wells with permit numbers 34067212200000 and 34067211080000 are operated by a plunger lift system. Permitted well number 3406721218000 is a reservoir pressure driven well. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

7 400 bbl. Production tanks 3 Producing wells 3 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Gas compressor unit. 1 Horizontal heater-treater. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1376675603 2013-08-16 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. Nomac Drilling Rig # 71 drilled a 17-1/2 inch diameter hole to 638 ft and set 628ft of casing. Schlumberger Cementing Service c/w 554 sks of class A cement. Was advised at 6 am that they would be done cementing at 11 am. I found them on schedule and the casing would not make up and job ran over.
