| On the day of my inspection I found that the access road up to the well had been mowed and cleared of vegetation to a point where it could easily accommodate an ATV. The field where the well is located was also mowed down and there were ATV trails around the perimeter. The well was still equipped for a plunger lift and the flow lines were connected at the wellhead. All of the valves at the wellhead were closed. There was no identification posted at the wellhead, and there were no production tanks found to be associated with this well. There were no gas or fluid leaks from the wellhead and no signs of contamination on the ground. I took updated digital photos of the well. I called the number in RBDMS for Atlas Noble and I spoke with a receptionist, who claimed that this well was part of a transfer package to Diversified Resources. I called the local number that I had recorded for Atlas; no one answered the call, but the voice greeting was for Diversified. I called Dale Smith, field superintendent for M&R Investments to ask about the well. He gave me the number for the superintendent in this area. I called and got no answer so I left a message. Dale also gave me the number for Tom Vosick, who is over all of M&Rs production wells in the state. I spoke with Tom, who claimed that they were meeting with DOGRM to devise a plugging program for all of the idle wells that they have acquired. He asked if I could send him a copy of the original compliance notice and he would get back to me.

They ran to the bottom of the tubing with their sand line, to an approximate depth of 6400 feet. Then they pumped through the tubing to assess the integrity. Howard was concerned with pulling the tubing out and running back in to set the bottom plug, so they brought Appalachian Well Surveys in to shoot the tubing off. They shot the tubing off at 6292 feet. Howard shut down for the day due to daylight, and they will pump the bottom plug in the morning.

The top section was very muddy and unpassable by vehicle. I saw no evidence of recent use to access the well. The well was equipped with a plunger lift system. The production casing valves were both closed. The tubing valve was closed. There was no gauge present to determine pressure. There was no identification posted at the wellhead. There were no leaks or spills at the wellhead. I searched the surrounding area and found no tank battery or production equipment to be associated with this well. The tank battery GPS coordinates stored in RBDMS plotted in an open area along TR 162 where I did observe a gas line riser. Although the annual production reports are up to date, no oil or gas production has been reported since 2001. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

| On the day of my inspection Howard Drilling established circulation and circulated 30 barrels through the tubing into their frack tank. Then they pumped a cement bottom plug with 76 sacks of Type 1 cement. Circulation was maintained throughout the plug. Once the plug was displaced they pulled 30 joints of the tubing to get above the Utica Shale. Appalachian Well Surveys tagged the bottom plug at 5600 feet. Then they perforated the 5.5 inch casing with 10 shots, spaced 1 foot apart, from 5460 to 5470. Howard Drilling ran tubing back in to get down to the perforations. They established circulation and pumped a 200 foot cement plug to isolate the Utica. Howard then pulled the tubing from the well.

| On the day of my inspection Appalachian Well Surveys shot the 5.5 inch casing off at 4150 feet. Howard Drilling pulled one joint and a sub to ensure the casing was free. They pumped a 300 foot cement plug through the Clinton sand with Type 1 cement. Howard Drilling started pulling the 5.5 inch casing from the well. I left the site because I was out of regular hours. I left instructions for the next plug with Larry Fetterman in case they made it that far before shutting down for the weekend.

| On the day of my inspection I found that Howard Drilling had set the 200 foot plug at the top of the Big Lime after I left the site on Friday 12/14/18. They were changing trailers to continue pulling the 5.5 inch casing up to the bottom of the surface casing for the next plug. They pulled up to 1137 feet and pumped a 200 foot cement plug at the base of the surface casing. Then they pulled up to 213 feet and checked the fluid level. The fluid level was down approximately 200 feet, so Howard pulled the remaining 5.5 inch casing from the well. They filled the well with cement from the surface. The cement was staying at the surface when I left the site. They said that they were going to move all the equipment off the site and leave the well open overnight to be cut and capped tomorrow morning.

| On the day of my inspection I found that Larry Fetterman with Diversified was at the site cleaning the township road. Larry said that the plugging crew cut the casing off  and capped it with the steel plate this morning. Howard Drilling had all equipment and debris removed from the location. The well site was rough graded and the access road had been scraped down to the clay. There was quite a bit of mud that had been pushed off to the sides of the access road and there were no water breaks. The culvert had been removed from the entrance. I told Larry that Diversified should get a dozer or mini hoe out here to get the access road fixed up with some water temporary water breaks to get through the winter. Larry said that he would contact Chuck Wood about getting the work done. This well has been plugged and the compliance notice is resolved.

| On the day of my inspection I found that the access road was still relatively clear and there were fresh tractor tracks visible. The field where the well is located was planted in corn. The well was still idle and there were no signs of any activity to bring the well into production. The valves were still closed and there were no tanks found for the well. No identification has been posted at the wellhead. A plugging permit was issued in December, 2017 but I have had no communication from M&R Investments with regards to the requested plug plan. I called Tom Vosick with Diversified to ask what they are doing with this well. He gave me the number for Drew Adamo and said that he was handling the plugging. I called Drew and he said that he had the permit in front of him. He admitted that the well was not on the radar as a high priority but he would try to bump it up on the list, due to the fact that it is almost two years past the remedy date.

There were no gauges on the well to determine pressure. There were no leaks or spills around the wellhead. I found no identification at the wellhead. There were no storage tanks or production equipment found to be associated with this well. A compliance notice was issued to plug this well on 3/15/16, which requested a written plugging schedule to be submitted within 30 days. No plugging schedule has been submitted, and there has been no contact by the owner/operator. I took updated photographs of the access road and wellhead.

1544734981 2018-12-13 Plug / Plug Back No Violations On the day of my inspection Howard Drilling established circulation and circulated 30 barrels through the tubing into their frack tank. Then they pumped a cement bottom plug with 76 sacks of Type 1 cement. Circulation was maintained throughout the plug. Once the plug was displaced they pulled 30 joints of the tubing to get above the Utica Shale. Appalachian Well Surveys tagged the bottom plug at 5600 feet. Then they perforated the 5.5 inch casing with 10 shots, spaced 1 foot apart, from 5460 to 5470. Howard Drilling ran tubing back in to get down to the perforations. They established circulation and pumped a 200 foot cement plug to isolate the Utica. Howard then pulled the tubing from the well.
1545056385 2018-12-14 Plug / Plug Back No Violations On the day of my inspection Appalachian Well Surveys shot the 5.5 inch casing off at 4150 feet. Howard Drilling pulled one joint and a sub to ensure the casing was free. They pumped a 300 foot cement plug through the Clinton sand with Type 1 cement. Howard Drilling started pulling the 5.5 inch casing from the well. I left the site because I was out of regular hours. I left instructions for the next plug with Larry Fetterman in case they made it that far before shutting down for the weekend.
1545057393 2018-12-17 Plug / Plug Back No Violations On the day of my inspection I found that Howard Drilling had set the 200 foot plug at the top of the Big Lime after I left the site on Friday 12/14/18. They were changing trailers to continue pulling the 5.5 inch casing up to the bottom of the surface casing for the next plug. They pulled up to 1137 feet and pumped a 200 foot cement plug at the base of the surface casing. Then they pulled up to 213 feet and checked the fluid level. The fluid level was down approximately 200 feet, so Howard pulled the remaining 5.5 inch casing from the well. They filled the well with cement from the surface. The cement was staying at the surface when I left the site. They said that they were going to move all the equipment off the site and leave the well open overnight to be cut and capped tomorrow morning.
1545146164 2018-12-18 Plug / Plug Back No Violations On the day of my inspection I found that Larry Fetterman with Diversified was at the site cleaning the township road. Larry said that the plugging crew cut the casing off and capped it with the steel plate this morning. Howard Drilling had all equipment and debris removed from the location. The well site was rough graded and the access road had been scraped down to the clay. There was quite a bit of mud that had been pushed off to the sides of the access road and there were no water breaks. The culvert had been removed from the entrance. I told Larry that Diversified should get a dozer or mini hoe out here to get the access road fixed up with some water temporary water breaks to get through the winter. Larry said that he would contact Chuck Wood about getting the work done. This well has been plugged and the compliance notice is resolved.

The culvert at the lease road entrance had been completely removed and the roadside drainage ditch re-established. The access road was still rough graded and no water breaks had been put in yet, but here were no problems with erosion.

1678687704 2018-07-30 Production Wells No Violations On the day of my inspection I found that the access road was still relatively clear and there were fresh tractor tracks visible. The field where the well is located was planted in corn. The well was still idle and there were no signs of any activity to bring the well into production. The valves were still closed and there were no tanks found for the well. No identification has been posted at the wellhead. A plugging permit was issued in December, 2017 but I have had no communication from M&R Investments with regards to the requested plug plan. I called Tom Vosick with Diversified to ask what they are doing with this well. He gave me the number for Drew Adamo and said that he was handling the plugging. I called Drew and he said that he had the permit in front of him. He admitted that the well was not on the radar as a high priority but he would try to bump it up on the list, due to the fact that it is almost two years past the remedy date.
