| On the day of my inspection I was accompanied by Supervisor Eric Conrad.  During the inspection we witnessed a Profrac frac crew onsite.  Stage 6/62 had been perforated but not fracked. Stage 6 plug set at 17068 feet, with bottom perforation at 17054 feet  and top perforation at 16934 feet. The 13.375x9.625 annular pressure was zero and the 9.625x5.5 annular pressure is zero. Casing pressure is 4476 psi.

0003694276 2019-01-17 Casing String Witnessed No Violations On the day of my inspection I witnessed the intermediate casing cement job. Patterson 571 drilled a 12.25 inch diameter open hole to a depth of 1995 feet ground level. KB is 25 feet and paperwork will say 2025 feet. Previous casing set and cemented was 13.375 inch diameter surface casing to a depth of 640 feet ground level. Wyoming casing ran 2010.4 feet of 9.625 inch diameter casing, j-55, 40 lbs/foot, with 27.4 stick up from ground level. Casing shoe at 1983 feet ground level. Casing was centralized with 46 centralizers. Shoe track is 44.5 feet long. Halliburton was onsite with 690 sacks of class A cement. Cement weighed with mudscale at 15.8 ppg and yield of 1.21. 42% excess ran in open hole. Cement displaced with 149 barrels of fresh water. 30 barrels of geland 15 barrels of water pumped ahead of cement. 22 barrels of cement returned to surface weighing 14.7 ppg, and floats held with 1.25 barrels back
0070418652 2019-05-24 Completion Testing No Violations On the day of my inspection I witnessed a Profrac frac crew onsite tied into the 10H and 210H wells. This well has not been pumped on to date. The 13.375x9.625 annular pressure is zero and the 9.625x5.5 annular pressure is 20 psi. Fourteen steel produced water tanks are on location within their own containment.
0443169959 2018-12-04 Drill / Deepen / Reopen No Violations On the day of my inspection I was accompanied by inspector Jeff Adams. We witnessed that Express Energy was onsite with two auger rigs. They are currently drilling this 26 inch diameter hole at a depth of 96 feet. They are planning on drilling to 120 feet and cementing casing on Thursday.
0946893596 2019-05-14 Completion Testing No Violations On the day of my inspection I was accompanied by Inspector Jeff Adams. A Profrac frac crew is on location tied into the 8H, 10H, and 210H wells. This well has not been opened up to frac yet. The 13.375x9.625 annular pressure is zero and the 9.625x5.5 annular pressure is zero.
1214852969 2019-05-06 Completion Testing No Violations On the day of my inspection I witnessed a Profrac frac crew onsite. Profrac is currently tied into the 8H and the 210H wells. This well has not been opened up for frac. There are fourteen 350 barrel produced water tanks on site, within their own containment. The 13.375x9.625 annular pressure was 0 psi and the 9.625x5.5 annular pressure was 0 psi.

1547639845 2019-01-16 Casing String Witnessed No Violations PAD ADDRESS: On the day of my inspection I witnessed the surface casing cement job. Patterson 571 drilled a 17.5 inch diameter open hole to a depth of 680 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. Wyoming Casing ran 669 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot with 4 foot stick up. Casing was centralized with 15 centralizers. Shoe track is 44 feet long. Halliburton was onsite with 675 sacks of class H neocem cement. Cement weighed with mudscale at 17 ppg and yield of 1.071. 50% excess ran in open hole. Cement displaced with 96 barrels of fresh water. 30 barrels of gel pumped ahead of cement. 26 barrels of cement returned to surface, and floats held with 1 barrel back to truck.
1664077479 2018-12-06 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augered a 26 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 216 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 30 percent excess ran. Cement was displaced with 39 barrels of fresh water. There no vac trucks on location so I was unable to confirm cement was returned to surface. There were good returns and expect that cement was returned to surface but cellar had 4 feet of water in it at the end of the job.
1977084373 2019-06-05 Completion Testing No Violations On the day of my inspection I was accompanied by Supervisor Eric Conrad.
