| The purpose of my inspection was to follow up on a phone call from Jeremy Stotts owner and operator of Eagle Drilling who informed me that there was a leak in the 4.5-inch diameter casing just above the 8.625-inch diameter casing head that he thought was being caused by a nearby injection well. On the day of my inspection accompanied by ODNR UIC inspector Dave Ball I witnessed production equipment included a 100-barrel steel production fluid storage tank, 25-barrel plastic brine storage tank, horizontal separator, and 2-inch diameter meter run with chart recorder and no chart all contained within an earthen dike. There was a 300-barrel mobile steel production fluid storage tank that was brought in to contain produced water. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 1.5-inch diameter tubing utilizing a plunger lift system as means of production, operator was not sure if plunger was installed. The well appeared to be providing domestic gas to two separate properties through a little joe regulator, drip and rotary style domestic meter and two 1-inch diameter plastic lines with the valves shut. There was a small hole in the 4.5-inch diameter production casing just above the 8.625-inch diameter casing head that had a band clamp installed to slow the flow of the leak, it was still leaking slowly around the band clamp. Jeremy informed me that he had received a call at approximately 12:30 am on 4/23/2019 from the landowner that the well was leaking. 1370273837 | 2019-04-25 | Production Wells | Violations Noted | The purpose of my inspection was to follow up on a phone call from Jeremy Stotts owner and operator of Eagle Drilling who informed me that there was a leak in the 4.5-inch diameter casing just above the 8.625-inch diameter casing head that he thought was being caused by a nearby injection well. On the day of my inspection accompanied by ODNR UIC inspector Dave Ball I witnessed production equipment included a 100-barrel steel production fluid storage tank, 25-barrel plastic brine storage tank, horizontal separator, and 2-inch diameter meter run with chart recorder and no chart all contained within an earthen dike. There was a 300-barrel mobile steel production fluid storage tank that was brought in to contain produced water. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 1.5-inch diameter tubing utilizing a plunger lift system as means of production, operator was not sure if plunger was installed. The well appeared to be providing domestic gas to two separate properties through a little joe regulator, drip and rotary style domestic meter and two 1-inch diameter plastic lines with the valves shut. There was a small hole in the 4.5-inch diameter production casing just above the 8.625-inch diameter casing head that had a band clamp installed to slow the flow of the leak, it was still leaking slowly around the band clamp. Jeremy informed me that he had received a call at approximately 12:30 am on 4/23/2019 from the landowner that the well was leaking. Jeremy informed me that when he arrived on location, he installed the band clamp, ran a 2-inch diameter plastic line from the 4.5 inch diameter casing to the 300-barrel tank, and excavated a ditch and built an earthen dike to the north side, down slope of the well. The produced water had made it down the hill side slightly but did not appear to have reached the stream to the north. I informed Jeremy to install 2-inch diameter plugs and a valve on the 8.625-inch |

Jeremy Stotts the owner and operator of Eagle drilling was on location and informed me that they had hauled approximately 320 barrels of produced water off of the 300 barrel steel production fluid storage tank in the past 24 hours, I informed Jeremy to keep an eye on the tanks and continue to haul water as often as needed. At the time of my inspection Jeremy was attempting to utilize the plunger lift system by flowing the tubing into the 100-barrel steel production fluid storage tank. At the time of my inspection the well appeared to be flowing approximately 1 barrel of produced water per minute through the 1.5-inch diameter tubing. I informed Jeremy if to continue to monitor the pressure on the 4.5-inch diameter casing, as well as the 8.625-inch diameter casing and to inform me of any changes. |

Upon inspection the well was observed to be equipped with a 4.5 diameter production casing string along with a 1.900 production tubing string. The 4.5 diameter production casing well head has a 2 diameter flow line attached to the casing head service port outlet. The 2 diameter flow line valve is open and the well is periodically flowing brine water into a temporally set 300 barrel horizontal tank. The fluid in the tank is being removed by H&L Water Service with a total of 700 barrel reported volume being removed since the reported day of the incident on 04/23/2019. Initially the well developed a hole in the 4.5 diameter production casing just above the well head slips at surface above ground level elevation. A temporary seal has been applied to the 4.5 casing over the hole and the casing was not leaking during the time of my inspection. Also the well owner initially reported the initial casing pressure on 04/23/2019 was 251 pounds pressure, the pressure on this day was observed to be zero pounds pressure. The tubing is equipped with a plunger lift system with the production valve in the closed position, the well will flow up the tubing if the valve is opened. No off site impact was observed on this day. The well owner has began to secure contractors and prepare to set a bottom plug in the well bore. The well owner is working to submit a plugging permit to the Division. |

Upon arrival the well was observed to be flowing brine from the 4.5 production casing well head 2 diameter outlet ports. To date Robert Hesson has hauled 13 water truck loads at a volume of 82 barrels each load. The well has flowed since 04/23/2019. Appalachian Wireline ran inside the 1.900 diameter production tubing with a primer cord shot to the depth of 5,550 , then the primer cord shot was deployed was deployed at 5,545 to split the tubing open. Cemenco then pumped 30 barrels of 10.1 pound per gallon brine water down the tubing with 6 barrels of mix being a salt water polymer gel followed by a 9.45 barrel cement mix consisting of 45 sacks of class A Neat cement displaced with 12.6 barrels of fluid. The pumping pressure during the event was 500 to 700 pounds pressure and the well was shut in upon the displacing of the cement plug. |
