| I was on location to check the status of drilling out the plugs in this well. Currently the Bishop Well Service rig is drilling ahead at 262. The returns include wood and metal. This would indicate they are inside the 7 diameter casing. They are going to perform a bit trip to install a new reaming bit to ensure proper penetration. They will continue this until to the top of the Big Lime. | | I was on location to check the status of drilling out the plugs in this well. | I was on location to witness the setting of the surface plug for this well. The well was drilled out to 312 with no indication that drill string was in the original borehole. It was decided that the rig would not drill any farther and set a surface plug to isolate the freshwater aquifer. 2.375 diameter tubing was run 312 into the well and 75 sacks of Class A 2% gel cement was pumped and displaced with 11.5bbls of freshwater. The tubing string was pulled from the well and the well was topped off with 20sks of additional Class A 2% gel cement. Cement was brought to surface and did not fall. This well has been plugged and the location will be restored. For a complete review of the plugging of this well please see the plug report. | | I was on location to witness the setting of the surface plug for this well. 0084722235 | 2015-03-09 | Plug / Plug Back | No Violations | I was on location to witness the cementing of a 5.5 diameter surface isolation casing in this well. The wellbore was milled out with a 7 diameter bit and 163 of casing was installed into the wellbore. Petroset pumped 130sks of Class A cement and displaced it to 130 with 3bbls of fresh water. The wellbore circulated and cement was returned to surface. The rig will wait on cement for 24 hours before they begin to drill out the well to the top of the Big Lime formation. This casing is to ensure freshwater isolation and proper cuttings return during the drill out. |
0111800409 | 2015-10-20 | Final Restoration | No Violations | I was on location to check the status of final restoration. The location has been reclaimed and vegetation has been established. All of the production equipment has been removed and there are no signs of the well leaking. A plugging report is on file for this well. At this time final restoration has been passed. |
0256119037 | 2015-03-26 | Plug / Plug Back | No Violations | I was on location to follow up to complaint #4982. The complainant stated that a fish kill has occurred at the lake located on his property. He is concerned that the lake has been poisoned by the plugging operation occurring at this well following the heavy snow melt. The well has been plugged and cemented on 3/25/2015. Currently the equipment on location is being rigged down and prepared for transport from the location. There were no visible spills or chemicals on location. The plugging of this well used only water based fluids and drilling muds. No saltwater or oil and gas producing geologic zones were penetrated during plugging. The water flowing into the lake of question did have a dark color which is a result of tannins leaching from decaying leaves. I met ODNR Wildlife Officer Mark Basinger on location to examine the fish kill. The fish appeared to have been dead for several days. While walking the bank live fish were seen swimming from the shore and geese were present on the lake. The fish killed included carp, catfish, bass and bluegill. After examining the fish kill located at the lake and several other spots downstream it was determined that the fish kill was not oil and gas related, but indeed a winter fish kill. A 3/19/2015 publication by the ODNR Division of Wildlife indicated fish kills would be common this spring due to the extreme winter that has occurred. At this time this complaint can be closed. |
0539886368 | 2015-03-03 | Plug / Plug Back | No Violations | I was on location to check the status of the drill out of the this well for re-plugging. The Bishop Wells service rig drilled from 267 to 275. Returns are consisting of steel and wood indicating that the drill bit is inside the previous wellbore. The rig will continue to drill tomorrow until it reaches a previous casing point inside the wellbore. |

1387178883 | 2014-10-29 | Plug / Plug Back | No Violations | I was on location to witness the re-plugging of this well by Bishop Well Services and Gonzoil. A cellar has been installed on location and cemented in around the 7 diameter casing at surface. 2 tubing has been found in the well and a plan is being devised on how to attach onto the 2 diameter tubing in order to pull it from the well. An attempt to log the well may be attempted if it is deemed possible to be able to enter the 2 diameter tubing. |
1409161687 | 2014-08-20 | Production Wells | No Violations | I was contacted by the landowner at 8059 Leaver Ave NW requesting an inspection of a pipe sticking out of the ground on his property. The pipe was above ground about 3 with a welded round cap on top and a valve. I contacted Mike Stanfar (330-351-4325) of Dominion to check if this pipe was associated with this well. A permit to plug the well has been issued on 8-14-2014. Dominion believes this pipe is the well in question, and are going to verify this before any work begins. They planned on plugging the well within a month and are going to work on access to the location from the landowners. |
1423586796 | 2015-02-09 | Plug / Plug Back | No Violations | I was on location to check the status of plugging this well. Bishop well service is currently drilling in the wellbore with a reaming bit at 224. They will drill to at least 600, to the top of the first plug. There they will cement in surface casing to ensure isolation of the freshwater strata. After surface casing is set they will drill out the remaining plugs until the top of the Big Lime. |
1427375629 | 2015-03-25 | Plug / Plug Back | No Violations | I was on location to witness the setting of the surface plug for this well. The well was drilled out to 312 with no indication that drill string was in the original borehole. The tubing string was pulled from the well and the well was topped off with 20sks of additional Class A 2% gel cement. Cement was brought to surface and did not fall. This well has been plugged and the location will be restored. For a complete review of the plugging of this well please see the plug report. |
