| The purpose of my inspection was a status check. The pad is named Wilson and it consists of 8 wells.  The well was equipped with a production tree style wellhead. Production valves were in the open position. The production equipment consisted of eight gas production units, eight 400 barrel tanks, two vapor recovery towers, and two combustors. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current.

| On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing.  At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing, and 13.375-inch diameter intermediate casing.  Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid storage tanks, and a combustor that was shared with the other producing wells on the pad.  There was secondary containment around the production fluid tanks, and the Gas Processing Units.  The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location.  There was a midstream valve site on the west side of the pad.  As well as a water impoundment to the south, right side of the lease road as you enter the pad.    Identification tags were present, and production was up to date.
