| I arrived on location to witness Express Energy Services set conductor casing on the WEHR UNIT 3H. Prior to my arrival Inspector Conrad had approved them to set the conductor to 80 feet. We set the conductor using 257 sacks of class A cement with 2% NaCl and about a half a pound of flake. The weight of the cement was 15.6 pounds and had a 1.18 yield with a 32.19 mix. We used a total of 43.35 barrels of fluid for displacement. When we finished setting the casing we had a 10 foot shoe. After we had finished setting the conductor we had about 18 barrels of cement come back to surface and 47 barrels of water come to surface giving us a total of 65 barrels total to surface.

0531973000 2016-10-20 Casing String Witnessed No Violations On the day of my inspection I witnessed that Frontier Drilling Rig #32 had drilled a 17.5 inch diameter hole to the depth of 1052 feet. Wyoming Casing Crew ran 1042 feet of 13.375 inch 54.5 pound per foot J-55 casing. C & J Energy Services was on site and gained circulation with 183 barrels of fresh water, pumped 25 barrels of gel, 10 barrels of fresh water and cemented casing with 1009 sacks of Class A cement weighing 15.6 pounds per gallon and a yield of 1.19 cubic foot per sack cement. Cement additives included 2% calcium chloride and .25 pound per sack of cello-flake. Cement weight was verified by pressurized mud scales. Cement was displaced with 155 barrels of fresh water and circulated to surface with good returns of 47 barrels. The plug was landed at 420 pounds per square inch and pressured to 920 pounds per square inch for 5 minutes and the floats held with 1bbl back. Mix water was tested revealing chlorides of 61 parts per million, pH 7 and 60 degrees.
0956215398 2016-12-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Frontier Drilling Rig #32 had reached total depth of their lateral with an 8.75 inch diameter bit at 16803 feet and was cleaning the wellbore with 10.5 pound per gallon (ppg) mud simulating 11.9 ppg. Frontier Drilling Rig #32 is planning on circulating 6 bottoms up before tripping out of the wellbore and running 5.5 inch diameter production casing with cementing to follow. I completed walk around with Antero Consultant Steve Guffey revealing proper containment was in place.

1500305558 2017-07-17 Drill / Deepen / Reopen No Violations Status check inspection found the well has defective 5.5 diameter production casing and is slated to be plugged. The remainder of the 6 wells on the pad are now in production. The company production men on site indicated the wells were dispersing the sand trap separators into a open top gas buster tank. I advised them to move the water truck pick up point to a safe area that was not downwind of the gas buster tank to avoid gas fumes from the engine from being introduced into the trucks engine while it was loading product.

The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. Halliburton crews are on site preparing to begin the well stimulation 5.5 diameter production casing toe preparations.
